Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference number is FM442762711675; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-15 and Defense DCN 20061219. This acquisition will be 100% small business set-aside; The Federal Supply Class (FSC) is 5830. The North American Industry Classification System (NAICS) code is 423450. The Standard Industrial Classification (SIC) is 5047. The business size standard is 500 Employees. This request for quotation consists of the following Amron International items or equal: Item 0001: Qty 1 EA, Amplifier 16 patient system PN: 8895-AMP-16 or equal. Item 0002: Qty 16 EA, Patient Entertainment Set PN: 8895-100 or equal. Item 0003: Qty 2 EA, Cable Assembly Ent to j-box 20 foot length PN: 8895-600 or equal. Item 0004: Qty 2 EA, Junction Box Ent outside PN: 8895-700 or equal. Item 0005: Qty 2 EA, Junction Box Ent inside PN: 8895-800 or equal. Contractors are asked to submit their delivery timeframe and their tax identification number with their proposal. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.204-7 Central Contractor Registration. FAR 52.212-1 Instructions to Offerors -- Commercial Items.
