- This is a combined synopsis/solicitation for commercial items requirements prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; a written solicitation will NOT be issued. This simplified acquisition procurement is also being accomplished in accordance with FAR Part 6.302- 1. The Department of Veterans Affairs (VA), Southern AZ VA Health Care System (SAVAHCS), Medicine and Primary Care Service Line, Cath Lab, 3601 S. 6th Avenue Tucson, Arizona, will award multiple Indefinite Delivery, Indefinite Quantity Consignment contract resulting from this solicitation. Consignment Agreement shall include the following items: (1) Drug Eluding Stents available in mono-rail and over-the-wire ranging from sizes: 8mm to 33mm in diameter and 2.5mm -4.0 mm in length; (2) Bare Metal Stents available in mono-rail and over-the-wire ranging from sizes: 2.00mm to 4.00 mm in diameter and 8mm to 28 mm in length. The contract period shall be for one (1) year from date of award with the Government having the option to exercise four (4), one (1) year renewal periods. Ordering requests will be made by the COTR on an as-needed basis. The applicable NAICS code for this requirement is 339113 size standard 500 employees. The Contractor shall provide a complete copy of the Offeror Representations and Certifications-Commercial Items (FAR 52.212-3 Nov 2006) with its offer or complete the annual representations and certifications electronically at http://orca.bpn.gov. Many of the provisions and clauses cited in this solicitation are incorporated by reference, FAR 52.252-2 (FEB 1998), to the appropriate section of the FAR and VAAR. A review of these provisions and clauses will be necessary for you to understand all aspects of the solicitation.
