High St, Rm. 540, Columbus, OH 43215-2495 DESCRIPTION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number AG-5540-S-0002 is issued as a Request for Quotes. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-15. The clauses and provisions referenced in this solicitation can be found in full text format at http://www.arnet.gov/far. This solicitation is 100% Set Aside for Small Business. The contract type will be firm fixed price. The associated NAICS code is 561720 and the small business size is $ 15 million. The following Department of Labor Wage Determination applies to this procurement: 2005-2421. The full text of the wage determination is included as an attachment. The United States Department of Agriculture, Farm Service Agency (FSA) announces a Request for Quote (RFQ) for Janitorial Services in the: Hardin County FSA Office, 12757 SR 309 West, Kenton, OH 43326. The leased property is 3,600 square feet with restroom space not included in that total square footage. Contiguous Counties, Auglaize and Van Wert, will require janitorial services, as well, solicited under separate cover as: AG-5540-S-0001 and AG-5540-S-0003, respectively. The Government will contemplate the award of multiple contracts to the same contractor, if it is in the best interests of the Government. When submitting a response, indicate the counties in which you have an interest. The Government anticipates the award of one or multiple Firm Fixed Price type of contracts resulting from this combined synopsis/solicitation. Responses should include the experience/qualifications of the contractor, personnel, equipment and services to perform the type of tasks described by this notice. High St. Rm. 540, Columbus, OH 43215-2495 STATEMENT OF WORK Services Required The contractor shall maintain the leased premises in a clean condition and shall provide supplies, labor, and equipment. Cleaning will be performed in 3,900 square feet of office space with included restrooms. Total carpeted space equates to 90%. The following schedule describes the level of services intended. Performance will be base on the County Executive Director?s (CED?s) evaluation of results, not on the frequency or method of performance. The CED will provide keys to the contractor. The service provider shall provide trash bags, cleaning supplies and equipment. Storage closet is available if contractor needs to store supplies and equipment. Cleaning is to be performed after tenant working hours unless daytime cleaning is specified otherwise. A. Weekly: Empty trash receptacles and clean ashtrays. Sweep or vacuum entrances, lobbies, corridors, and offices. Sweep and damp mop or scrub toilet rooms. Damp mop and spray buff all resilient floors in restrooms, main corridors, entrances, and lobbies. Clean all toilet fixtures and replenish toilet supplies such as toilet paper, paper towels, and soap. Complete vacuum of all carpet areas. Scrub or wet mop hard floors. Clean drinking fountains. Police sidewalks, parking areas, driveway, grounds and shrubbery. Dispose of all trash and garbage generated in or about the building. Dust horizontal surfaces that are readily available and visibly require dusting. Remove carpet stains. B. Monthly: Damp wipe toilet wastepaper receptacles. Buff resilient floors in office space. Spot clean all wall surfaces within 70 inches of the floor. C. Three Times Annually (March, July & November): Vacuum or dust all surfaces in the building over 70 inches from the floor. This includes Venetian blinds and light fixtures. Vacuum all drapes in place. Strip, wax, and buff resilient floors, use non-slip floor wax. D. Twice Annually: Wash all glass surfaces, Venetian blinds, roller shades, and light fixtures. Services listed below require 7-day written advance notice to the government: E. Every six months: Wash all interior and exterior windows and other glass surfaces. Strip and apply four coats of finish to resilient floors in toilets. Strip and refinish or spot clean carpet in main corridors and other heavy traffic areas. F. Annually: Wash all Venetian blinds and dust 6 months from washing. Vacuum or dust all surfaces in the building within 70? (178 cm) of the floor, including light fixtures. Strip and refinish floors in offices and secondary lobbies and corridors. Clean carpets in corridors and lobbies. As required: The vendor shall provide the government with written notice 7 days prior to performing unscheduled service and maintenance.
