Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number is W912K3-07-T-0012 is issued as a Request for Quote (RFQ). This request for Quote is due by February 6, 2007, 4:00pm (local time). The RFQ will result in a firm fix ed price contract. This solicitation is 100% set aside for small business. The North American Industry Classification System (NAICS) code for this acquisition is 621210. The United State Property and Fiscal Office of the Washington Army National Guard is soliciting quotes to procure the following supply or services: CLIN 0001: Dental Exams. 10-11 February 2007. Quantity 330 . at Camp Murray Tacoma Washington. Unit Price $________ Total $________ .See statement of work for CLIN 0001 below. STATEMENT OF WORK FOR 10-11 FEB 07 DRILL WEEKEND CAMP MURRAY 1. Need personel only to use clinic exam station equipment in Bldg 34 Camp Murray, Tacoma on 10-11 Feb 07 with anticipated number of exams of 200-240 on Saturday and 70 -90 on Sunday . 2. Services are to include annual dental exam per AR 40-501 with bitewing films. Panograph x-rays will also be taken if the soldier has none in their military dental record or the x-ray on file is no longer current. Generally, x-rays taken within the las t five years are considered current provided there have been no significant changes in the mouth. 3. Workday will be a maximum of 10 hours/day. Anticipated start time is 0830 both days. 4. If the service provided uses mobile van or trailer services, the service provider should NOT expect that an adequate power hook up will be available, and that a generator will be required. 5. If the soldier has a current dental exam that is 6-9 months old or older, a new dental exam will be done. 6. A copy of the sign-in log of all soldiers seen will be deliver in person, by fax or by mail to the Deputy State Surgeon. The sign in log must include the following: Soldiers name, SSN Dental Classification Is there a current pano film in record? Yes/No (If a pano film is taken that day, mark yes) 7.
