- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quote s are being requested and a written solicitation will not be issued. The combined Synopsis/Request for Quotations (RFQ) Number is W81K00-07-T-0068 using Simplified Acquisition Procedures under FAR Subpart 13.5. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-14, dated 22 November 2006. It is the contractors responsibility to be familiar with applicable clauses and provisions. The associated North American Industrial Code Sy stem (NAICS) code is 511130 and the size standard is 500 employees. This acquisition is 100% set-aside for small businesses. Contract Line Item Numbers (CLINS) : 0001, EMT-B, Emergency Care and Transportation of the Sick and Injured, hardcover book, 9th Edition (R), ISBN: 0763744069, 5,000 each, to be shipped in increments of 500 every two weeks; 0002, EMT-B, Student Workbook, 9th Edition, ISBN: 0763748579, 5,000 each, to be shipped in increments of 500 every two weeks; 0003, Combat Medic Field Reference Manual, ISBN: 0763735639, 5,000 each, to be shipped in increments of 500 every two weeks. Quotes shall include all shipping and handling charges (FOB Destination) for delivery to AMEDD Center and School, 3151 Scott Road, Bldg 2841, Room 1413, Fort Sam Ho uston, TX 78234-6138. The commercial item FAR clauses and provisions applicable to this requirement are 52.212-1, 52.212-2, 52.212-3, 52.212-4, and 52.212-5. FAR provisions 52.212-1, Instructions to Offerors Commercial Items (Sep 2006) and 52.212-4, C ontract Terms and Conditions Commercial Items (Sep 2005), are incorporated by reference. Contractors shall include with their quote, past performance information on at least two other contracts in which they provided the same or similar items. Past per formance information shall include the contract number, company/agency name and address, and point of contact name with phone number and email address. Award will be made to the lowest priced quote, with satisfactory past performance that takes no excepti on to the specifications identified in CLINS 0001-0003. Small business concerns responding to this combined synopsis/solicitation shall include with their quote, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certification s Commercial Items (Nov 2006) and DFARS 252.212-7000, Offeror Representations and Certifications Commercial Items (Jun 2005). Firms submitting a quote for this requirement shall complete only paragraph (k) of FAR 52.212-3, if the firm has completed th e annual representations and certifications electronically at the Online Representations and Certifications Application (ORCA) website at http://orca.bpn.gov.
