Description
The United States Patent and Trademark Office (USPTO) intends to issue a fixed price, best value order for the Manufacture, Delivery and Installation of signs for the Global Intellectual Property Academy (GIPA). Contained herein: 1. Specifications 2. Evaluation Criteria 3. Schedule 4. Requirements 5. Vendor Checklist 1.0 Specifications a. Art Panels Description: Contact Contract Specialist for drawing specifications. Quantity: Nine d. Room Signs Description: Contact Contract Specialist for drawing specifications. Quantity: Five e. Cafeteria Sign Description: Contact Contract Specialist for drawing specifications. Quantity: One f. Podium Signs Description: Contact Contract Specialist for drawing specifications. Quantity: Four g. Reception Desk ID Sign Description: Contact Contract Specialist for drawing specifications. Quantity: One 2.0 Evaluation Criteria The following factors shall be used to evaluate offers. Both factors will be equally weighted in consideration for the winning bid. A. Price B. Ability to meet schedule and have all signs fabricated and installed before Grand Opening 3.0 Schedule 1/08/07 PTO to send out RFQ for signage elements 1/11/07 at noon, PTO to receive questions from bidders 1/12/07, PTO to respond to all bidders with answers to questions 1/16/07 at noon, PTO to receive bids 1/17/07, PTO to make award for bids 1/18/07, Signage fabricator to start creating shop drawings and to fabricate signs 2/21-23/07 - Signage installation; installation to occur during day Note: The Global Intellectual Property Academy intends to have its Grand Opening on February 26th, 2007 and would like all sign elements to be in place before that date. If the installation schedule as outlined cannot be met, the bidder must submit a complete installation schedule detailing which signs will be completed and installed by 2/23/07 and when other signs will be installed. Any signs that cannot be fabricated and installed before 2/23/07 must be installed after 5 pm on weekdays or weekends. 4.0 Other Requirements 4.1 Delivery and Service: The Manufacturer shall be responsible for the delivery of all items as specified, and shall provide the following services: At the onset of the project and prior to delivery, verify building entrance, loading dock and building exit dimensions to ensure truck accessibility. Coordinate loading dock access and all unloading activities with USPTO Project Manager. Provide advance notice to any changes in the delivery schedule. 4.2 Inspection and Acceptance Inspection and acceptance of the signage is the responsibility of the Government 4.3 Installation: Protect walls and floors of all corridors, building areas, staging areas, elevators, and rooms where furniture is to be installed. Damage will be repaired at no additional cost to the government. Unload product and deliver to designated staging areas. Uncrate and unwrap the product. Remove all boxes, wrappings, and other miscellaneous materials and return to the truck. No trash or packing material may be left at USPTO or discarded in the building's trash containers or dumpsters. Install product in each location, per plans provided. Damages to building property will be documented during the walk-through by the building owner's representative and submitted to the USPTO Project Manager and Installation Contractor on a damage report. The Manufacturer will provide, for all installation personnel assigned to the USPTO project, the following information at least two business days prior to any installation: name, address and social security number. All personnel working on the USPTO premise will be required to wear visible identification at all times. General building access guidelines and/or security requirements will be issued by USPTO before the first delivery. Personnel may be required to sign in and out each time when entering or leaving the premises. 4.5 Codes and Regulations The Manufacturer is responsible for ensuring that all installations shall comply with all applicable ADA, state, local, and federal building codes, local ordinances and regulations. 4.6 Installation Inspection and Acceptance Upon completion, the Manufacturer shall perform a walk through with the USPTO Project Manager or designate and prepare a report and punch list, noting all items damaged, incorrectly installed, missing, or with incorrect finishes. Upon 100% completion, the Manufacturer shall prepare a statement of acceptance of work and services performed under this delivery order, confirming that work has been performed in compliance with the specifications and the USPTO schedule, for signature by the USPTO Project Manager. Upon receipt of the signed statement, and an original invoice, the USPTO will issue payment to the Manufacturer. 5.0 CHECKLIST This information must be submitted to USPTO in order to be considered for evaluation.
