- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. Solicitation W912JF-07-T-0003 is a Request for Quote (RFQ). Contractors may obtain a copy of the combined synopsis/solicitation, Statement of Work (SOW), and any attachments at the following v ia http://www.nationalguardcontracting.org/Ebs/AdvertisedSolicitations.asp?SolType=Simplified%20Acquisition through Federal Acquisition Circular 2005-06. This acquisition is 100% set-aside for small business. All responsible sources may submit a quotation, which shall be considered by the agency. The NAICS for this solicitation is 621111. The Government intends to make a single award of an indefinite delivery/requirements type contract with firm fixed unit prices. Award will be made to the responsive/respo nsible offeror providing the lowest unit cost and is located in the Fort Chaffee geographic area. Only quotes received from physician(s) within a 10 mile radius of Fort Chaffee, Arkansas will be reviewed. The contract period shall consist of one base year and four option years for renewal. The estimated quantities for the base year and four option years are as follows: Base Year 40 EA Physical Examinations, including systems review, posturing, past medical history, occupational history; 5 EA Complete Bloo d Count (CBC); 5 EA Urinaysis (UA); 5 Chemistry Panel (CM); 5 EA Chest X-Ray (Two Views); 5 EA Blood Lead with ZPP; 5 EA PSA. Option Year 1 - 40 EA Physical Examinations, including systems review, posturing, past medical history, occupational history; 5 EA Complete Blood Count (CBC); 5 EA Urinaysis (UA); 5 Chemistry Panel (CM); 5 EA Chest X-Ray (Two Views); 5 EA Blood Lead with ZPP; 5 EA PSA.
