- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W912JF-07-T-0000 is a Request for Quote (RFQ). Contractors may obtain a copy of the combined synopsis/solicitation, Statement of Work (SOW), and any attachments at the followin g via www.nationalguardcontracting.org through Federal Acquisition Circular 2005-06. This acquisition is 100% set-aside for small business. All responsible sources may submit a quotation, which shall be considered by the agency. The NAICS for this solicita tion is 621320. The Government intends to make a single award of an indefinite delivery/requirements type contract with firm fixed unit prices. Award will be made to the responsive/responsible offeror providing the lowest unit cost and is located in the C amp Robinson geographic area. Only quotes received from Optometrist offices within a 10 mile radius of Camp Robinson, Arkansas will be reviewed. The contract period shall consist of one 12-month base year and four 12-month option periods for renewal. The e stimated quantities for the base year and four option years are as follows; Base Year - 100-Exams, Option Year 1 100 exams, Option Year 2 100 exams, Option Year 3 100 exams, and Option Year 4 100 exams.
