- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference number is F3Z3456171A400; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-14 and Defense DCN 20061109. If two (2) or more responsible and technically acceptable small businesses respond, this acquisition will be 100% small business set-aside; if not, it will be unrestricted.; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 Employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. This request for quotation consists of the following items: Item 0001: 1 ea Endourology Table System, OSI (Orthopedic Systems, Inc.) part# ETS OR EQUAL. This system consists of mondular table advanced control ?I? base, DC, OSI part# 5803 or equal. Endo urology top w/ Gimbals (400 lbs), OSI part# 5944 or equal. Endourology table equipment cart, OSI part# 5880-02 or equal. Lithotomy leg holders (supine), OSI part# 5054 or equal. Prone spreader bars, patient safety straps, T-pins, H-frames, lateral position head board, lateral arm board accessory mount, supine operating table top, foot section/patient transfer board, accessory adapter set, clark sockets, prone position head board, inflatable chest/kidney pad, inflatable hip/chest pads, prone arm boards (pair), patient cure kits, disposable uro catch bags, uro catch bag frame w/ clamps. The price should also include shipping to Travis AFB, CA 94535, uncrating, installation, staff inservice (one day), 3 operator?s manuals, 2 maintenance manuals, and 2 years parts and labor warranty. AN OR EQUAL BID WILL BE ACCEPTED, but is subject to approval from the government to confirm it will fulfill the requirement. The following provisions and/or clauses apply to this acquisition.
