- This posting constitutes Amendment 003 to RFQ50903-0001-7, Bag in Box Juice Beverage Supply/Service for the Federal Correctional Institution located in Fort Worth, Texas. The following paragraph under the title, "Submission of Quotations" has changed to: SUBMISSION OF QUOTATIONS: On letterhead or business stationary, the following information is required: (1) Contractors DUNS Number (2) Quoted pricing per gallon for the following Contract Line Item Numbers (CLIN): CLIN 001 - Non-carbonated (ratio of 15:1 or 19:1) beverages in 5 gallon BIB containers. The flavor selection must include passion fruit, fruit punch, lemonade, sweet tea, unsweet tea, and orange (sweetened with a blend of natural and artificial sweeteners, Vitamin C enriched) per contract year. Annual estimated quantity is 6500 gallons. CLIN 002 - Carbonated (ratio 5:1) beverages in 5 gallon BIB containers. Annual estimated quantity is 3500 gallons. Example format for your quote: CLIN 001 Product Ratio (i.e, 15:1, 19:1,) Base Year: CLIN 001 x $ __________________ Per gallon (of finished product) = $_____________________ per year. A price quote for CLIN 001 and CLIN 002 must be provided for the Base Year, Option 1, and Option 2. (3) A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (MAR 2005) if different from entry made to the Online Representations and Certifications Application (ORCA) site located at orca.bpn.gov (4) A list of all flavors that will be available to the government under a resultant contract. (5) Proof of contractors ability to meet insurance requirements. (6) A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (MAR 2005) if different from entry made to the Online Representations and Certifications Application (ORCA) site located at orca.bpn.gov (4) A list of all flavors that will be available to the government under a resultant contract. (5) Proof of contractors ability to meet insurance requirements. (7) Quoters are hereby notified that the Government may request a sample of the finished product be provided at the ratio stated in their quote. Product samples will be provided at no additional cost to the Government. Sealed Quotes must be received at the FCI Fort Worth, 3150 Horton Road, Fort Worth, Texas 76119, no later than 2:00 pm local time on Friday, December 1, 2006. Quoters wishing to hand-carry or deliver quotes must provide written notification to the Contracting Officer, no less than 24 hours in advance of arrival, so that arrangements may be made with base security personnel.
