Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Statement of Work must be requested of the Contract Specialist, Shannyn M. Scassero at shannyn.scassero@amedd.army.mil. All questions must be submitted electronically to the Contract Specialist no later than the close of business 6 DEC 06. This requirement establishes manning support for the Armed Forces Institute of Pathology (AFIP)/Office of the Armed Forces Medical Examiner (OAFME) during large-scale or long-term contingency opera tions at the Port Mortuary, Dover AFB, Dover, DE. The services requested will support US Soldiers, Sailors, Airmen, Marines, Coast Guardsmen and civilians serving in the Department of Defense mission. The location of this duty is The Charles C. Carlso Ce nter for Mortuary Affairs, Dover AFB, Dover, DE. The work schedule will be established by the Armed Forces Medical Medical Examiner or his designee and will be based on the number and arrival times of human remains. The Contractor must have a great degre e of flexibility in the days and times he/she is available for duty and must be able to adjust their schedules on short notice (48 hours). This requirement will request the services the services of a General Dentist (1 Full Time Equivalent), Dental Assist ant (1 FTE), Autopsy Assistant (1 FTE), Photographer (1 FTE), Diagnostic Radiologic Technologist (.75 FTE) and Forensic Pathologist (.50 FTE). All personnel listed above are determined to be key personnel. Overtime is authorized. It is anticipated that the Government will be awarding a T&M contract consisting of one base year with four, one-year options (Base Year 1 JAN 07 - 31 DEC 07; Option Year 1 1 JAN 08 - 31 DEC 08; Option Year 2 1 JAN 09 - 31 DEC 09; Option Year 3 1 JAN 10 - 31 DEC 10; Option Year 4 1 JAN 11 - 31 DEC 11) . Award will be based on best value to the Government. The contract start date is 1 JAN 07. The Best Value Evaluation will be based on the following criteria: 1) Personnel Qualifications-List the qualification of each candidate including degrees; licenses; certifications; advanced training including forensic dentistry and/or experience in perfo rming forensic dental identification; skills with computer programs and databases; knowledge of OSHA guidelines, HIPAA policies and guielines. Please include documented knowledge of the processes and procedures utilized by the Armed Forces Institute of Pa thology, Office of the Armed Forces Medical Examiner and specific to the operations at tyhe Dover Port Mortuary at Dover AFB, Dover DE. 2) Past Experience- Specific experience related to the Dover Port Mortuary operations and the processes and procedures utilized through the Armed Forces Institute of Pathology, Armed Forces Medical Examiner; 3) Management Plan-A plan specifying how the Contractor will perform the required services, meet the hours of operation that are extremely erratic based on 24 hour pe r day, seven day per week operation and met overtime requirements in the event overtime is necessary.; 4) Technical Skills- Ensure the Contractor personnel have the required technical skills, appropriate certifications, degrees, training and state licenses to perform the work associated with each position 5) Past Performance-A past performance questionnaire will be provided for submission. The evaluation scale is as follows: Excellent-Contractor exceed the minimum requirements of the STatement of Work and can provide experienced personnel 100% of the time as required. The majority of personnel are familiar with the processes and procedures utilized by the Armed Forces Institute of Pathology, Office of the Armed Forces Medical Examiner and specific to oper ations at the Dover Port Mortuary, Dover AFB, Dover, DE; Satisfdactory-Contractor meets the minimum requirements of the Statement of Work and can provide experienced personnel 100% of the time as required; Unsatisfactory-Contractor cannot meet the minimum requirements of the Statement of Work and cannot provide experienced personnel 100% of the time as required. Cost will be evaluated, but the above listed criteria are determined to be more important. The closing date for this requirement is 13 DEC 06 at 12:00 PM.
