Description
Provide professional architectural and engineering services necessary to accomplish the design for the Pharmacy IV Room at the VA Medical Center at North Chicago, IL. The Scope of work for the Pharmacy IV, USP 797 Compliance is as follows: Provide A/E design services for the remodeling of an area in the Outpatient Pharmacy (1A-136D) in Bldg 133. The design shall bring the IV Sterile prep/Compounding room into compliance with USP 797. The latest update to USP 797 shall be used in the design of this area. A/E services under this contract include, but are not limited to the following: (1) Design development plans, three copies for review and due in 30 calendar days after receiving the Notice to Proceed. (2) Three copies for review of the 90% drawings, and one copy of Specifications and preliminary cost estimate due in 30 calendar days after approval of the design development plans. (3) Working drawings and specifications due 15 calendar days after approval of the 90% submittal. Drawings are to be Autocad (no later then 2005) dwg files on CD-R disc and the specifications in Microsoft Word (not later than 2003, SP1) on CD-R disc. Also, provide one set of reproducible drawings on 5 mil polyester film. (4) A cost estimate, three copies due with the working drawings and specifications. (5) Review and approve all submittals. Provide engineering support for all RFI?s and Change Orders. (6) Provide for three post bid meetings or job inspections (other than the Final inspection) as requested by the Government. (7) Conduct a final inspection and deliver a report within ten days. (8) Incorporate installed variations and contract changes into working drawings, and provide ?as-built? Autocad files. Since this is a sterile compounding room for IV solutions; mechanical expertise is essential. All 8a A/E firms within a 100 mile radius of the VA Medical Center at North Chicago, IL and who meet the requirements listed in this announcement, are invited to submit two (2) copies of the completed Standard Form (SF) 330 no later than noon on December 21, 2006. PLEASE NOTE: TWO copies of packages are to be sent to the attention of Elaine Rahn, Contracting Officer at the following address: VA Medical Center/GLAC; 5000 W. National Ave; Milwaukee, WI 53295-0005. NAICS code 541330 applies. This requirement is set aside for an 8(a) firm. No bid packages are available. A Statement of work is attached; however, this is not a Request For Proposal (RFP).
