- This synopsis announcement is being issued as a Request For Quotation (RFQ) to a single source. Upon request, a copy of the RFQ on the Standard Form DD 1449 will be provided. The RFQ number is H94002-07-Q-0004 and has a response due date of November 24, 2006. This RFQ will be a 100% set-aside to small businesses. FOB Destination shall be 16401 East Centretech Parkway, Aurora, CO 80011. The contract will result in a Firm Fixed Price contract. The Contractor shall be registered in Central Contractor Registration (www.ccr.gov) before award can be made. Send quotations to Ms. Suzanne A. Garrett, Office of the Assistant Secretary of Defense Health Affairs, TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, CO 80011-9066, by email at suzanne.garrett.ctr@tma.osd.mil or by facsimile to (303) 676-3987. Facsimile quotes should be followed by signed, hardcopy forwarded to the address stated above. The quotation submitted by the contractor against this requirement shall clearly state the following: Request For Quotation number; Time specified for receipt of offers; Name, address, telephone number of the Offeror?s point of contact; Price and identification of any discounts applied; Acknowledgement of all quotation amendments (if applicable); Copy of completed Representations and Certifications; And the quote shall be in the same order as provided in this RFQ. This RFQ is for the single source purchase of a Xerox Maintenance Warranties containing base billing costs and per copy for the Xerox copiers listed below which are currently owned by TRICARE. All copiers have the capabilities for copying, network printing, and scan to email functions. The Contractor shall provide full service to maintain the copiers in optimum operating condition for the period of 1 December 2006 through 30 November 2007. All expendable supplies are included at no charge except staples and paper. Xerox products provided by the contractor must be Xerox compatible Parts and toners should be new and not refurbished or recycled. The following copiers require warranty, base billing, and cost per copy for black and white and for color respectfully as designated. CLIN 0001 ?
