- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotes (RFQ). Submit written offers (oral offers will not be accepted), on RFQ FM488761787400. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14 and Defense Acquisition Circular 91-13. This procurement is being issued as Unrestrictive. The North American Industry Classification System (NAICS) Code is 334510 and the size standard of 500 employees applies to this procurement. The requirement is to procure Second Look 300 with installation and training by ICAD Inc, Qty ? 1 ea, brand name or equal. Included are the following options ? Second Look Viewer, PowerLook, Modality worklist Query, DICOM Send, Second Look 300 Diamond Extended Service. FOB Destination to Luke Air Force Base, Arizona 85309. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The evaluation will be based on low price, and technically acceptable offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and 52.212-3 Alt I, Offeror Representations and Certifictions?Commercial Items.
