Offerors are required to complete and include a copy of the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (Feb 1999) with their proposals. FAR clause 52.212-4 Contract Terms and Conditions Commercial Items (May 1999)applies to this acquisition. FAR clause 52.212-5 Contrac t Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items (May 1999) applies with the following appliciable clauses from paragraph (b). DFARS 25212-7001 Contract Terms and Conditions Required to Implement Statutes of Executive Orders Appliciable to Defense Acquisition of Commercial Items (Jan 1999)applies to this acquisition with the following clauses appliciable from paragraph (b). DFAR 252.225-7036 North American Free Trade Agreement Implementation Act.DFAR 252.204-7004 Required Central Contractor Registration (May 1998) In addition to the listed FAR provisions and clauses, all offerors must provide a contractor identification code which is currently the Dun and Bradstreet Data Universial Numbering System (DUNS)number. Al l FAR provisions and clauses, except the Year 2000 Compliant clause, may be reviewed and/or obtained from the General Services Administration's Federal Acquisition Regulation webite at internet address http://www.arnet.gov/far/. The following factors shall be used to evaluate proposals; technical, price and past performance. Vendors should provide technical information that adequately describes th e item being offered. The pricing should provide unit price and total price. Vendors are required to submit past performance information, including points of contacts, on the last three contracts for similar type equipment if available. The closing date is hereby extended to 21 Nov 2006. Offers must be received prior to 4:00 pm CDT 21 Nov 2006 to be considered timely.
