Description
This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in Federal Acquisition Regulation Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Reference Number F2Y3FC6286A100 is issued as a request for quotation (RFQ). The incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-13. This solicitation is set-aside for small business. The associated Standard Industrial Classification Code (SIC) is 8744 and the North American Industry Classification System (NAICS) is 561210. The Small Business Size Standard is 500 Employees. CLIN 0001 ? Aerobic Instruction in accordance with Statement of Work, Quantity 720 each. Period of Performance is 1 January 2007 ? 31 December 2007. Place of performance is the McChord AFB Fitness Center, 62 SVS/SVMP, 729 5th ST McChord AFB WA 98438. Lott, Contracting Officer, Phone (253) 982-1082, Fax (253) 982-3656, Email: Elke.lott-02@mcchord.af.mil STATEMENT OF WORK-AEROBICS The contractor shall provide fully qualified instructors to teach a variety of safe, effective and motivational aerobic classes at the McChord Air Force Base Fitness Center Annex, Bldg 726. The class sessions shall include the following aerobic type activities: high and low energy aerobics, strength and flexibility training, circuit training, spinning, yoga, and step/bench aerobics, etc. The program outline shall be designed so that students perform warm-up exercises as well as cool-down exercises, emphasizing safety to prevent musculoskeletal injuries and overexertion. The program outline shall adhere to leadership and exercise programming philosophies recommended by accredited fitness and exercise professionals (American College of Sports Medicine, American Council of Exercise, etc.). 1. Contractor must provide enough certified instructors to meet the requirements of the contract. Certification must be from any of the below listed organizations, or any other nationally recognized associations in accordance with AFI 34-266, Para 3.3. Before an instructor can begin instruction a copy of their certification(s) shall be provided to the Contracting Officer. American College of Sports Medicine American Council of Exercise (formerly IDEA) Institute For Aerobics Research Aerobics and Fitness Association of America Ashtanga Yoga Teacher Training 2. Instructors must hold a current CPR Card and First Aid card issued by the American Red Cross or American Heart Association with copies to be submitted with the certification required in the preceding paragraph. 3. The Contractor provides certified aerobic instructors to conduct various aerobic classes that take place Monday through Friday. Schedule for days, times, and types of aerobic classes being held are available upon request. 4. The Fitness Center will provide exercise mats and exercise steps, sound equipment, facility and utilities. Contractor shall provide all batteries for microphone, music tapes/CD and instructional material. Contractor shall be responsible for all fees and royalties in the use of copyrighted material. 5. The Base Fitness Center Athletic Director reserves the right to change days and times according to customer demand, and may add other aerobic type classes with the mutual agreement of all parties involved. 6. The total estimated quantity of aerobic class sessions for the period of performance is estimated at 720. 7. Class sessions shall be 60 minutes in length, with a maximum class size of 40 participants. 8. The Fitness Center will issue a log sheet to the instructor. The instructor will confirm and annotate the attendance count for the session, initial for that session, and ensure that the log sheet is returned to Fitness Center personnel. 9. Individual session cancellations may be made directly to the contractor by the Base Fitness Center Athletic Director or the Procuring Contracting Officer for any reason. The Government will endeavor to minimize the number of cancellations and, when necessary, will attempt to provide maximum prior notification. Sessions cancelled 24 hours or more prior to the scheduled start of the session being cancelled will not be billable. Sessions cancelled less than 24 hours prior to the scheduled start time will be considered as Emergency cancellations and will be counted as a session performed for billing purposes. 10. The Contractor will invoice monthly at the per session price for all sessions performed during the month. Class sessions will not be held on the following official holidays: New Year?s Day Martin Luther King Jr.? Birthday President?s Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day (There will be no classes from 17-31 December 2007, inclusive)
