Description
PERFORMANCE WORK STATEMENT (Part 2 & 3) Dobbins ARB, GA 20 September 2006 2. DEFINITIONS/ACRONYMS. 2.1. DEFINITIONS. 2.1.1. CONTRACTOR. The term as used in this contract refers to the prime contractor. 2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, or terminate contracts, and make related determinations and findings on behalf of the Government. Note: The only individual who can legally bind the Government. 2.1.3. CRITICAL RESULTS. Laboratory values for tests that by their nature (either below or above the reference range) are significantly abnormal and for which immediate medical attention may be required to prevent significant patient morbidity or mortality. The Contractor?s laboratory shall publish a list of tests with results it deems to be in the critical range, and the special notification procedures it shall follow. 2.1.4. C-2 SECURITY REQUIREMENTS. These are found in the National Security Agency ?Orange Book? (DoD 5200.28-STD, DoD Trusted Computer System Evaluation Criteria). Basically, computer system processing is sensitive, but unclassified information, and shall have the appropriate security features. 2.1.5. QUALITY ASSURANCE. The Government procedures used to verify that services being performed by the Contractor are performed according to acceptable standards. 2.1.6. QUALITY ASSURANCE PROGRAM (QAP). All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements. 2.1.7. SHIPPING FORM. Form(s) used to initiate shipping actions. 2.1.8. SPECIMEN. A quantity of human blood, serum, tissue, excreta, patient body fluid, or environmental specimen provided for test purposes. 2.1.9. SUBCONTRACTOR. One that enters into a subcontract and assumes some of the obligations of the primary contractor. 2.1.10. TRANSFER TUBE. Any tube used to collect, transport, transship, and store samples under terms of the PWS. 2.2. ACRONYMS. GOVERNMENT-FURNISHED ITEMS. The Government will: 3.1. Perform specimen collection (e.g., phlebotomy services) to obtain acceptable specimens from patients. 3.2. Provide biologic specimens that have been collected and processed (centrifuged and separated, labeled, and preserved) per the Contractor?s published pre-analytical requirements, and in accordance with good laboratory practices (GLP). 3.3. Order required tests via approved software or online service, and provide the Contractor with test requests, lists, or work documents with required patient demographics and ordering information. For these automated sites, the Government shall not complete additional Contractor?s forms, data entry or test requisitions in the Contractor?s automated laboratory system. For each test requisition, Government personnel will provide: 3.3.1. Patient?s full name. 3.3.2. Patient?s Social Security Number (SSN). 3.3.3. Dobbins ARB Accession Number. 3.3.4. Date of Specimen Collection. 3.3.5. Date Specimen Shipped. 3.3.6. Test(s) Requested/Code and Name. 3.3.7. Ordering Physician?s Name. 3.3.8. Ordering Remarks/Comments (if clinically appropriate). 3.3.9. Patient?s Age or Date of Birth. 3.3.10. Patient?s Sex.
