Description
Modification 00001: This modification is being issued to change the requirement from a sole source to a competitive procurement. This modification also changes the incumbents name from Bert Design to Stephen Greene, 1806 Jackson Street, Baltimore, MD 21230. This is a Request for Quotations. Quotes are being requested and a written solicitation will not be issued. Requirement number 713-7-923801 is issued as a Request for Quotations. The Centers for Medicare and Medicaid Services (CMS), has a requirement for a contractor to provide technical document preparation and computer formatting and electronic preparation services for (4) issues of the Health Care Financing Review: Winter 2006-2007, Volume 28, Number 2; Spring 2007, Volume 28, Number 3, Summer 2007, Volume 28, Number 4; and Fall 2007, Volume 29, Number 1. The period of performance is approximately December 15, 2006 through December 14, 2007. The Statement of Work and Deliverables are attached. The contractor must be able to provide a combination of technical, graphic, and artistic capabilities necessary to produce layout material for a scientific econometric journal similar to the Review. This output shall include electronic, camera copy, Adobe portable document format (PDF) files, and Adobe Illustrator graphics. The contractor must be capable of preparing and maintaining Quark Express style sheets numbering approximately 50, which allow the rapid formatting of covers, spine, title pages, unique feature logos, by-lines, disclaimers, text, data tables, charts, footnotes, technical notes, table notes, custom legends suitable for black and white output, sources, references, reprint requests, etc. In addition, the contractor must be capable of preparing custom-drawn, one of a kind graphics and also scientific formulae using Greek, algebraic, calculus, and statistical character sets. This is a firm fixed price procurement using simplified acquisition procedures. This procurement is set aside for small business concerns. The North American Industry Classification System Code (NAICS) is 541430. A firm is a small business for the purpose of this requirement if its size standard is $6.0 million. Responses may be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov or in paper no later than 3:00pm on November 17, 2006 to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd, C2-21-15, Baltimore, MD 21244. Responses shall be stapled without use of elaborate covers and shall not include brochures, newsletters, or other materials uniquely created for this procurement. All responses shall be no more than 10 pages. A page of paper printed on both sides is considered as two pages (if using double-sided pages, the front side would be page 1, the reverse side would be page 2). The response shall include the following: 1) Contractors capabilities overview; 2) Proposed technical approach to meeting the requirement; 3) Past performance information (relevant experience to perform the required services); and 3) Price of the deliverables. All responses will be evaluated on a best value basis using the following evaluation criteria: 1) Technical and Management Approach, the likelihood of effectively meeting requirements; 2) Past performance, the relevance and quality of prior performance; and 3) Price, the amount, realism and consistency of the evaluated price. Interested contractors are hereby advised that contractors must be currently registered in the Central Contractor Registry (CCR).
