- Contractor shall furnish qualified medical personnel and support staff, equipment, medications, and supplies for providing chronic hemodialysis, peritoneal dialysis services, and ancillary services on an as-needed basis at their facility to veteran beneficiaries receiving their primary physician care at the Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106. NAICS Code is 621111 with a small business size standard of $9.5 million. Solicitation is unrestricted and a multiple award is anticipated.
