Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91243-07-T-0002. This combined synopsis/solicitation has been set-aside for small busines set-aside only. The NACIS code is 621210. This is the statement of work: 1.0 SCOPE: This Statement of Work (SOW) covers annual dental exams for personnel of the 1195th Transportation Company on 18-19 November 2006 to be completed on at the Kearney Army National Guard Armory, 5710 Airport Road, Kearney, NE. This SOW identifies the purpose of the work to be performed by the contractor and provides a description of the work to be completed and any required outcomes. The contractor shall provide qualified personnel and material to perform the following services in accordance with this SOW. 2.0 PURPOSE: The purpose of this project is to complete required annual dental examinations to assign a dental classification of the risk of having a non-combat dental emergency during deployment. Dental Readiness is fundamental to maintaining unit readi ness and reducing non-combat casualties during deployment. 3.0 REFERENCE: 3.1 AR 40-501 Standards of Medical Fitness 3.2 AR 40-35 Dental Readiness and Community Oral Health Prevention 3.3 Department of the Army Personnel Policy Guidance (PPG) 4.0 SERVICES: The contractor shall provide all management, labor, material, equipment, certifications and supplies necessary to provide mobile dental services using dental vehicles or portable dental examination equipment. This service will be delivered 18-19 November 2006 at the Kearney Army National Guard Armory, 5710 Airport Road, Kearney, NE. This service will provide mandated dental examinations for 100 soldiers required to meet dental readiness and deployability standards to ensure mission capabilit y of the Nebraska Army National Guard. 4.1 TASKS: 4.1.1 Dental Examinations: The Contractor shall conduct Dental Examinations in accordance with AR 40-501 and AR 40-35 to include bitewing radiographs and panographic x-rays for 100 assigned personnel. The Contractor shall ensure that 27 0 soldiers are examined during an 8 hour day. Dental Exams will be conducted in the Kearney Nebraska National Guard Armory by using either portable dental equipment to include digital bitewing capability and digital panograph equipment or a Self-Contained Mobile Dental Vehicle. The contractor will ensure that all soldiers have a dental panograph on file in their dental record. The contractor shall also ensure adherence to the requirements of the Occupational Safety and Health Administration (OSHA) and appl icable Federal Regulations. 4.1.2 Documentation: The Contractor will ensure that all Soldiers have a complete dental record to include military dental record jacket, DA Form 5570 (Health Questionnaire envelope), privacy act statement, SF 603/603a and/or DD Form 2813, HIPPA no tice of privacy practices, and supporting radiographs. The panographic radiograph will adequately represent the current oral condition of the Soldier. The Contractor shall complete data entry of dental classification and panograph completion into the MEDPR OS/MODS system. 5. GENERAL INFORMATION: 5.1 Quality Control: The Contractor shall adopt a quality assurance program that monitors all service activities and ensures the highest quality customizable dental services available. It is the responsibility of the ARNG to review and approve dental tr eatment plans before treatment is performed based on the guidelines provided by NGB. 5.2 Hours of Operation: Soldiers will be processed from 0730-1700 hours on 18 November 2006 and 0730  until completion on 19 November 2006. 5.3 Security Requirements: The base security main gate will be notified of team arrival. All contract employees will be required to show identification at the main gate upon arrival. 5.4 Reporting: The contractor shall provide a final report to include the number of soldiers screened a by-name roster of soldiers identified as a Dental Class III.
