Description: The Department of Veterans Affairs, North Texas Health Care System has a requirement for Community Based Outpatient Clinic (CBOC) that shall provide primary care to eligible veterans in the Texas counties of Collin, Cooke and Denton. The contractor shall provide for the delivery and management of continuous primary, preventive and mental care. The contract shall provide health care providers, medical facilities, medical equipment and supplies, short-term pharmaceutical prescriptions and administrative support. Compensation shall be made through monthly capitation payments for each patient in accordance with the terms and conditions of the solicitation. The due date for this solicitation is not later than 2 pm CST on December 6, 2006. The solicitation is unrestricted.
