- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YU0-06-T-0041 and is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through F ederal Acquisition Circular 2005-13 and Defense Change Notice 20061004. This procurement is unrestricted. The North American Industry Classification System Code (NAICS) is 423450 and the Size Standard is 500. Contract line items: Item 0001 Headlight Camer a System (1) Each; Item 0002 Single Port Light Source (1) Each; Item 0003 Digital Video Recording System (1) Each; Item 0004 Pre-Formatted DVDs (1) Package/100 DVDs per package; Item 0005 Footswitch (1) Each; Item 0006 Monitor (1) Each 21 inch Flat Screen Color LCD Monitor; Item 0007 Video Cart (1) Each; Item 0008 Freight. Please acknowledge all requirements on proposals. The minimum requirements are: Watts: minimum of 300W; L/S Warranty: minimum of 3 yea rs; Lamp Type: Xenon or equivalent product; Lamp Warranty: minimum of 650 hrs. Dimensions: Length: minimum of 14 inches; Width: minimum of 12 inches; Height: minimum of 5 inches; Weight: maximum of 20 lbs. Color Temperature: minimum of 6000 Degrees K; Safety: minimum Type BF or Type CF; Turret Options: ACMI-WOLF-OLYMPUS-STORZ (AWOS) cabl e combination or equivalent; Lamp Age Meter: Yes; Type: Digital; Noise Range at 3 feet: minimum 59 63 db; Light Output; Cable Lumens: minimum of 680; Headlight Lumens: minimum of 352. Offerors must provide brochures/literature of the items being offered along with their proposal, if applicable. The technical literature submitted needs to clearly communicate the offerors understanding and compliance wi th the minimum requirements as set forth in the specifications or any alternative proposal submitted in response to this solicitation. Any salient characteristics of items must be documented in the proposals. The product shall be delivered FOB Destination within 6 weeks after issuance of the purchase order to: Madigan Army Medical Center, 9902 Lincoln St MAMC Annex, Tacoma, WA 98431-1110. Contractor shall provide all materials, labor, and transportation n ecessary to accomplish this requirement. 52.212-1 Instructions to OfferorsCommercial Items applies to this acquisition. The Government will issue a purchase order resulting from this solicitation to the responsible vendor conforming to the solicitation t hat will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: A) Compatibility with existing equipment in the Department of Surgery/Ophthalmology Services Madigan Army Medical Center. Currently, the Department is utilizing the Microlux Headlight Camera System. B) Interchangeability with existing equipment and parts in the Department of Surgery/Ophthalmology Services Madigan Army Medical Center. C) Technical capability mus t meet the minimum requirements stated above. D) Price. The Government reserves the right to award to other than the lowest priced offer. Vendors shall include a completed copy of the provisions at 52.212-3, Offeror Representations and Certifications-Commercial Items, and its Alternate I, and DFARS 252.212-7000, Offeror Representations and Certifications Commercial Items, with its quote. 52.212-4, Contract Terms and Conditions-Commercial Items, and Addendum to 52.212-4(c)Unilateral Modifications: 1. The Contracting Officer may unilaterally reduce and de-obligate any quantities not supplied/performed at the end of the base contract period , and at the end of each option period if applicable. 2. The Government may make unilateral modificatio ns considered administrative in nature. These include, but are not limited to, changes in the accounting and appropriation data, payment and issuing office addresses, and other corrections that have no effect on the terms and conditions of the contract. ARMY ELECTRONIC INVOICING INSTRUCTIONS (FEB 2006) also applied to this acquisition. 52.212-5, Contract Terms and Conditions Required to Implement Statues Or Executive Orders-Commercial Items, applies to this ac quisition. The additional FAR clauses cited in paragraph (b) which are applicable are: 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, and 52.225-13. 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items is applicable.
