- Contractor shall provide all labor, materials, equipment, transportation and other items and services necessary to perform refuse removal/disposal and the pickup of cardboard at the Harrison Village Commissary, IN. This Request for Quotation will be released on or about October 23, 2006. This Request for Quotation will be issued an Unrestricted basis. Only written requests will be accepted, facsimile or email acceptable and must contain the following information: Request for Quotation Number, requestor's name and address, email address, business size, telephone number, facsimile number DUNS number and CAGE code.
