Description
This is a revision to the Combined Synopsis/Solicitation, published on 03 Oct 2006, Reference No. F1D4526226A400NewbornScreening. The following verbiage was changed: From: The Period of performance shall be from date of contract award through 30 Sep 07. To: The Period of Performance shall be from the date of contract award through 30 Sept 07 and four (4) 12-month option periods from 01 Oct -30 Sep. The response date for submission of offers shall not change as a result of this amendment. ----------------------------------------------------------------- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. Reference Number F1D4526226A400NewbornScreening is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-04 and DCN 20050621. This is an unrestricted action. Number note 1 is applicable. The NAICS code is 621511 and the business size is $12,500,000.00. This combined synopsis/solicitation contains one line item as follows: Newborn Screening Services. Provide 1200 Newborn Screenings in accordance with the following Performance Work Statement, for a period of one (1) year. Services will be provided at the 79th Medical Group, Malcolm Grow Medical Center, Bldg. 1050 Andrews AFB, MD 20762. PERFORMANCE WORK STATEMENT GENERAL REQUIREMENTS: The contractor shall provide all services, materials and equipment necessary for the service of Pediatric Newborn Screening tests, Malcolm Grow Medical Center Laboratory located at Andrews AFB, MD to ensure dependable and reliable operation of the equipment. The scope of work performed under these specifications includes the furnishing of all supplies, shipping and testing requirements to assure continued operation at their designed efficiency and capacity. THE CONTRACTOR SHALL PERFORM THE FOLLOWING FUNCTIONS: NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (12-OCT-2006); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
