Description

The Contractor shall provide 0.25 (6 hours per week estimate) part time qualified Home Based Behavior Psychologist to provide home care and disease management consultative services in accordance with the terms and conditions of the contract. The contractor shall provide clinical psychologists (PhD, Psy.D) with a Kentucky psychologists license and training in chronic disease management. Designation as a Health Service Professional Provider (HSPP) is desirable but not essential. Contract Psychologists shall be of a quality meeting or exceeding currently recognized national standards such as established by the Joint Commission on Accreditation of Healthcare Organization (JCAHO) and the standards of the American Hospital Association. The services are provided for the VAMC, Louisville, KY 40206. This procurement is a unrestrictive, small acquisition procurement, commercial item contract. No web based offers shall be accepted. All amendments and other documents will be posted on the above website without further notice by the VA.
