PAST PERFORMANCE QUESTIONNAIRE (3 PAGES) AND CLIENT AUTHORIZATION LETTER (1 PAGE) REMOVED 4. SCENARIO FOR SOLICITATION FOR BASE CAMPS (2 PAGES) 5. WAGE DETERMINATION 05-2009 REV (01) (14 PAGES) D. OFFEROR QUESTIONS AND ANSWERS RECEIVED ARE ATTACHED. E. ATTACHMENT 01, SUBMISSION OF PROPOSAL, EVALUATION CRITERIA AND BASIS FOR AWARD, has been revised. The following general areas have been revised: 1) Para 3a(5), number of awarded BPAs has been changed from ?5? to ?3?; 2) Para 3b ? the page limitation has been changed from ?30? to ?20?; a limit of 5 graphics pages has been added; 3) Factor 1 has been changed from Past Performance to Scenario and additional criteria has been added; 4) Factor 2 has been changed from Scenario to Past Performance. The requirement to submit Past Performance Questionnaires has been removed. Offeror must provide a list of references and additional information; 5) Factor 3 has been changed from Business Proposal to Management Team and evaluation criteria has been provided; 6) Factor 4 is the Business Proposal. Pricing will be firm-fixed-price; 7) Basis for Award has been changed to reflect all four factors; 8) Para 5 ? a. Offeror is to submit original and 6 copies of proposal; b. Proposal due date has been extended to October 3, 2006, 4:30 p.m. CST; c. ATTN Line for proposal address has changed; d. Technical submittal includes: Scenario Response, Past Performance References, and Management Team Response F.
