- Provide back-up audiology coverage during times of peak need and leave periods during the course of the contract in accordance with all terms and conditions of the contract for the VA Salt Lake City Health Care System. The contractor will provide the following services: Bone Conduction, Air Conduction, Speech SFT & Speech Discrimination, Hearing Aid Orientation, Immittance, Hearing Aid Evaluation, ABR Counseling, Diagnostic Testing, Nonorganic tests, Hearing Aid Check, Electroacaustic Test of Hearing Performance, Earmold Impressions & Real ear measurement of hearing and performance. Contractor is required to have state license and certification for the American Speech Language & Hearing Assoc (ASHA, CCC-A). The contract will run from January 1, 2007 through December 31 2008 with four one year options to extend the term of the contract. The contract will utilize simplified acquisition procedures in accordance with Commercial Items Part 12 and in accordance with Part 15, contracting by negotiation. The contract will be an IDIQ contract. The Issue date is O/A 10-12-2006. The BOD is O/A 11-10-2006.
