Description

This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a separate written solicitation will not be issued. The solicitation number is 598-0153-06 and is issued as an RFQ (Request for Quotation). This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-12 effective Aug. 4, 2006. The NAICS Code is 315223 and business size standard is 500 employees. The proposed acquisition is a 100% small business set-aside. DESCRIPTION/SPECIFICATIONS: The contractor is to provide 2,700 to 3,000 men?s and women?s style polo shirts. Exact quantities, sizes and delivery dates to be determined at a later date. The shirts should be 100% cotton, short sleeve, no pocket, weight: 7.1oz., color: royal blue, with embroidery on the left chest. The embroidery to include in block white lettering ?CENTRAL ARKANSAS VETERANS? with a red star and one point of the star making the white letter ?V? for ?VETERANS? followed underneath by white block lettering ?HEALTHCARE SYSTEM?. The stitch count for the embroidery is up to 10,000 stitches. A sample of the required logo design may be acquired through request by email to sheila.reed@med.va.gov or by selecting the attachment included. SCHEDULE OF SUPPLIES/SERVICES AND PRICE/COST: Pricing should include all costs associated with the setup, production and completed delivery per shirt as FOB Destination pricing.
