UNFUNDED REQUIREMENT. FUNDING IS NOT AVAILABLE AT THIS TIME. IF FUNDING IS NOT AVAILABLE BY 30 SEP 2006 THIS SOLICITATION WILL BE CANCELLED IN ITS ENTIRETY. This is an amendment to the combined synopsis/solicitation for Solicitation Number: Reference-Number-AerobicsSep2206 posted 15 September 2006. This amendment is to add the Performance Work Statement for the synopisis/solicitation stated above. STATEMENT OF WORK AEROBICS INSTRUCTOR 1. SCOPE OF WORK 1.1 Contractor shall conduct and administer all aspects of the exercise portion of the Weight Management and Fitness Enhancement Programs, held at the Base Fitness Center (Bldg 9510), Elmendorf AFB, AK. 1.2 Programs shall consist of an aerobic type workout and include but are not limited to: step aerobics, high/low impact, modified; slide aerobics, water aerobics, pre-natal instruction, yoga, and spinning. 1.3 Projected schedule of services: NOTE: Classes will be performed using the following schedule as an example. This schedule is meant to be a guide. The total number of classes offered must equal the quantity of classes identified in the solicitation. Aerobics Oct - Mar classes at 0600, 1130, 1715, and 1830 hrs Monday ? Friday, Saturday at 1130 Apr - Sep classes at 0600, 1130, and 1730 hrs Monday ? Friday, Saturday at 1130 Spinning Mon, Wed, Fri at 1100 and 1730, Sat 1100 Water Aerobics Mon, Wed, & Fri at 1800 Yoga Fit Mon 0930, 1130, 1830; Wed 1130, 1830; Fri 0930 These times are subject to change based on the needs of the active units. Contractor will be notified 30 days in advance of any changes in class times. 1.4 The following schedule will be used for Holidays, Military Down/Family Days: 1.4.1 Thanksgiving Day and Christmas Day: no classes will be offered. 1.4.2 Federal Holidays (not mentioned above): one aerobics class, 1130, and one spinning class will be taught. 1.4.3 The contractor does not have to provide instructors on other days as identified by the Fitness Center due to closure or special circumstances when notification is given 24 hours prior. 2. PERSONNEL 2.1 If the contract is awarded to an agency outside the greater Anchorage area, the contractor will provide an Aerobics Contract Manager (ACM) that is certified in Aerobics and Fitness Association of America (AFAA) Primary Group Exercise Certification and has at least three (3) years teaching experience. The Contractor and/or ACM must be able to personally mentor new instructors in safety guidelines and methods of instruction. 2.2 The contractor shall designate in writing to the Fitness Center Director, the names and telephone numbers of the ACM and an alternate (when ACM is not available). These designated individuals shall meet with Government personnel to discuss immediate problems, should they arise. 2.3 The contractor and/or ACM work under the authority and direction of the Fitness Director. Complaints and requests for all issues are to be addressed utilizing the chain of command. 2.4 The contractor?s instructors will possess, as a minimum, certification by one of the following national fitness organizations: Aerobics & Fitness Association of America (AFAA), or American Council of Exercise (ACE). Other certifications such as Johnny G/Mad Dogg Spinning, or Yoga Fit, are required, if they are to teach those type of classes. In addition, all instructors (including substitutes) must possess a current Cardiopulmonary Resuscitation (CPR) card. All certifications must be valid prior to employment, and copies presented to the fitness center before providing instruction. Copies of the certification(s) will be maintained on file at the fitness center. If a certification expires on any instructor, instruction will cease until certification is renewed. 2.5 The contractor is reminded that off-duty, active military personnel hired may be subject to permanent change of station (PCS), change in duty hours, recall, and deployments. Their absence does not constitute a valid excuse for non-performance under this agreement. 2.6 The contractor and/or ACM shall be financially responsible for any damage caused to fitness Center equipment while it is under their control (i.e. broken keys/locks, stereo equipment, microphone/headset, etc.) to include the repair/replace, maintenance, shipping and handling of damaged equipment. 3. EMPLOYEE IDENTIFICATION 3.1 Contractor personnel shall present a neat appearance and be easily recognizable while in the Fitness Center, which should be accomplished by wearing distinctive athletic attire. 4. SPECIFIC DUTIES 4.1 ACM 4.1.1 Provide to the Fitness Center, required certifying accreditation in physical conditioning, weight management, or certification from recognizable institutes for himself/herself and any other individuals who may instruct classes. As a minimum, the individuals must be certified by a nationally accredited organization stated in paragraph 2.4 above. 4.1.2 Provide a master program schedule, in a monthly calendar format, depicting aerobic instructor name for the designated type of class. This schedule must be submitted to the fitness center director ten (10) days prior to the effected month. 4.1.3 The ACM is responsible to interview prospective instructors and verify certifications are valid and current. 4.1.4 The ACM will provide the Fitness Center with a complete list of instructors, to include telephone numbers, as well as their qualifications within thirty (30) days after award of contract, and to keep it current. 4.1.5 The ACM will limit the number of classes taught by one instructor to no more than two classes per day. 4.1.6 The ACM will train instructors on proper usage of stereo equipment prior to equipment being used. 4.1.7 The ACM shall ensure each instructor incorporates standardized and safe warm up, stretching and exercise techniques. A minimum of two (2) target heart rate checks will be performed during each class, as well as a safe cool down period to conclude the cardio segment of each class. 4.1.8 ACM or representative will attend monthly meetings with the Fitness Center staff to receive any changes and class scheduling. 4.1.9 ACM shall conduct a monthly meeting with all instructors to keep them informed of facility/base policy changes or any other information needed to ensure a quality program. 4.1.10 ACM will ensure an instructor is present for scheduled classes. If for some reason, a scheduled instructor cannot make a scheduled class, the ACM must provide a qualified substitute to teach the same type class as scheduled. 4.1.11 ACM shall ensure instructors notify on-duty fitness center staff of any injuries or accidents associated with aerobic patrons. Should ambulatory services be needed, the fitness center shall notify the proper agency for assistance. The instructor is required to assist the fitness center staff in the completion of an accident report after the conclusion of the incident. 4.1.12 ACM will contact the Contracting Officer to obtain the necessary information required to request issuance of a base pass to gain entry onto the base during contract period of performance. 4.2 Instructor 4.2.1 Instructor shall arrive 10-15 minutes prior to the scheduled class start time in order to sign out keys, headset, and retrieve stereo equipment for each class. In addition, instructors will provide their own music, either by cassette or compact disc for each class. 4.2.2 Instructor will set up and take down all stereo equipment and ensure all equipment is returned to its proper place and that the exercise area is clean after the class and keys returned to the front desk. 4.2.4 Equipment malfunction or problems with the class shall be reported to the Fitness Director or representative by the end of the class. 4.2.5 Instructors will record an accurate head count for each class performed using forms provided by the Fitness Center. 4.2.6 Instructors will be safety conscious and will begin each class with a short safety briefing, followed by an effective class that is positive and motivating to the students. 4.2.7 Instructors will provide specialized instruction for students demonstrating high and low impact options as well as beginning, intermediate, and advanced techniques in a progressive building format. 5. GOVERNMENT RESPONSIBILITIES 5.1 The Government shall provide, without cost to the contractor, the utilities necessary to accomplish the tasks specified in this Statement of Work, including electric, fire protection and 911 services. 5.2 The Government shall provide a designated area suitable for aerobics instruction. In addition, the Government shall provide steps, mats, weights, bands, balls, spinning cycles, and a sound system. 5.3 The Fitness Director will train the contractor and/or ACM on proper usage of stereo equipment prior to being used. 5.4 The Government will monitor the quality of instruction provided by all instructors as well as to ensure instructors are eligible to participate under this statement of work. 5.5 The Fitness Director may cancel or add aerobic classes (upon the availability of funds) due to mission requirements, insufficient class attendance or substandard contractor performance. The Director will provide the contractor an eight (8) hour notice prior to cancellation, at a minimum.
