Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number M00146-06-T-3367 is hereby issued as a Request for Quotation. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-11 and Defense Acquisition Circular 98-78. This procurement is under the North American Industry Classification System code 423850, with a small business size standard of 500 employees. This requirement is for a firm fixed priced contract, brand name or equal, for the following item: Item 0001, 8 each Survivair Air Packs. Survivair CBRN Panther Air packs with fully wrapped carbon fiber cylinders with locking collar, built in compass pass device with therm alarm, over shoulder buddy breather, analog gauge, pathfinder firefighter locating system beacons, medium panther 20 20 plus CBRN face masks with head nets and small talk plus. The cylinder duration time shall be 30 minutes with 4500 PSI pressure. Part Number 969011C. Manufacturer: Survivair, Cage Code 30RK3. Note: If you are quoting on an equal, you must include literature for the brand quoted for evaluation purposes. Quote must contain unit price, extended price, terms of payment, company name, address, name of individual quoting, phone and fax numbers. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR 52.212-1, Instructions to Offers - Commercial Items (JAN 2006). FAR 52.212-1 Addendum. The following additions/changes are made to FAR Provision 52.212-1, which is incorporated by reference: The following provision is hereby incorporated by reference: 52.211-6 Brand Name or Equal (AUG 1999). 52.211-14 Notice of Priority Rating For National Defense Use (SEPT 1990). 52.212-2, Evaluation - Commercial Items. The Government will award a contract resulting form this solicitation to the responsible offeror whose quote, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Technical capability to meet required specifications. 2. Price 3. Delivery 4. Warranty FAR 52.212-3, Offer Representation and Certifications-Commercial Items (MAR 2005), Alternate I (APR 2002). A completed copy of this provision shall be submitted with the offer. FAR 52.212-4, Contract Terms and Conditions Commercial Items (SEPT 2005). FAR 52.212-4 Addendum. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (JUN 2006), applies with the following clauses applicable for paragraph (b); (12)(i) 252.225-7036 Buy American Act-North American Free Trade Agreement Implementation Act-Balance of Payment Program (JUN 2006) (ii) Alternate I (JAN 205) (17) 252.232-7003 Electronic Submission of Payment Requests (MAY 2006) and (20)(i) 252.247-7023, Transportation of Supplies by Sea (MAY 2002).
