- This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. Solicitation RFQ 548-06-514. This solicitation is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The associated NAICS # 423450 with a size standard of 100 employees. The VA Medical Center located at 7305 N. Military Trail, West Palm Beach, Florida, has a requirement to purchase three (3) Ultrasound Scanners. Prospective contractor?s shall submit a written quote, providing pricing on the following equipment: 1) Brand Name or Equal Site Rite V ultrasound scanner with 5 to 10 MHz linear probe to include combination AC Adapter/Battery Charger, two (2) single needle guide kits, IFU and a standard roll stand with auxiliary battery - three (3) each; 2) medical printer with side arm bracket three (3) each; 3) operator manual three (3) each; 4) service manual one (1) each. Successful quoter must be registered with the Central Contractor Registration (CCR). You may register in CCR on-line at http://www.ccr.gov. The following FAR provisions and clauses are applicable to this solicitation: 52.212-1, Instruction to Offerors - Commercial Items, 52.212-3 Offeror Representations and Certifications ? Commercial Items, 52.212-4 Terms and Conditions ? Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ?
