- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation W91YTV-06-T-0331 is being issued as a Request for Quotation. The solicitation document and incorporated clauses are those in effect through Federal Acquisition Circular 2005 -12. This acquisition is not set aside for any small business concerns. The applicable NAICS code for this acquisition is 334516 and the small business size standard is 500. Eisenhower Army Medical Center, Fort Gordon, GA, has a requirement for the foll owing blood bank equipment: CLIN 0001, 1 each 5 head high volume blood tube sealer. A suitable system will create a seal in under 3 seconds through either automatic or foot pedal control. Operator must be able to select from left to right or right to le ft sealing and adjust the seal pattern. Genesis BPS Catalog Number 428-SE850RS, Brand Name or Equal. CLIN 0002, 2 each manuals for supplied sealer. Terms are FOB Destination-Eisenhower Army Medical Center, Building 300, East Hospital Road, Fort Gordon, GA, 30905-5650. Required delivery date is 30 days after receipt of order. The provisions of FAR 52.212-1, Instructions to Offerors-Commercial applies to this acquisition without addenda. The provisions of FAR clause 52.212-4, Contract Terms and Condition s-Commercial Items, apply to this acquisition with addenda that direct the use of Wide Area Workflow to submit electronic invoices if available.
