- This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6 and FAR FAC 2005-12. This announcement constitutes the only solicitation, and is issued as a Request for Quotation. A WRITTEN SOLICITATION WILL NOT BE ISSUED. This is an Unrestricted acquisition. This is a firm-fixed price procurement procured using commercial procedures. Please see attached Statement of Work. Solicitation Number is FA4809-07-Q-7027. The NAICS code 323121 applies to this solicitation. The size standard is 500. The item description is as follows: Base Library Book Rental Service for 1 Oct 06 ? 30 Sep 07. CLIN 0001: Adult Book Lease Plan, Plan Index #GB7-6 Or Equal, Qty: 12, Unit of Issue: Months, Unit Price $_______. CLIN 0002: Children?s Book Lease Plan, Plan Index #PC-3 Or Equal, Qty: 12, Unit of Issue: Months, Unit Price $_______. CLIN 0003: Audio Book Lease Plan, Plan Index #AL-08 Or Equal, Qty: 12, Unit of Issue: Months, Unit Price $______. SEE THE FOLLOWING STATEMENT OF WORK: For Adult Books - Allowed 75 book points per month - Allowed to have up to 1,300 books in the collection at any given time - Yearly new book allowance is 600 - Allow Library to send back books at any given time - Allow Library to select what books they want each month - Company will send out Best Sellers List (new book selections) in advance (at least 30 days) so Library can select their choices - If a new book becomes damaged within the first three months, company will replace with a new book free of charge - If a book becomes damaged within six months, the company will replace with a used book at not charge - Library is allowed to retain eight books per month for their permanent collection. For Children?s Books - Allowed 90 book points per month - Annual Point Allowance is 1,080 points - Allow Library to send back books at any given time - Allow Library to select what books they want each month - Company will send out Best Sellers List (new book selections) in advance (at least 30 days) so Library can selet their choices - If a new book becomes damaged within six months, the company will replace with a used book at no charge - Library is allowed to retain eight books per month for their permanent collection. For Audio Books - Allowed 240 book points per month - Annual Point Allowance is 2,880 points - Allow Library to send back books at any given time - Allow Library to select what books they want each month - Company will send out Best Sellers List (new book selections) in advance (at least 30 days) so Library can select their choices - If a new book becomes damaged within the first twelve months, company will replace with a new book free of charge. The following clauses and provisions apply to this acquisition FAR 52.212-1 Instructions to Offerors-Commercial Items; The RFQ should be submitted and shall contain the following information: 1) RFQ number, 2) time specified for receipt of offers, 3) name, address, and telephone number of offeror, 4) terms of any express warranty, 5) price and any discount terms, 6) Delivery Date, 7) a completed copy of the representations and certifications at FAR 52.212-3 Alt 1 Offeror Representations and Certifications-Commercial Items (see FAR 52.212-3(j) for those representations and certifications that the offeror shall complete electronically); Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration, (IAW FAR 4.1201 prospective contractors must be registered in the Online Representative & Certification Application (ORCA) at http://orca.bpn.gov ) 7) Period for acceptance of offers - The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation, 8) Late offers - Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is ?late? and will not be considered.
