Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation W91YTV-06-T-0320 is being issued as a Request for Quotation. The solicitation document and incorporated clauses are those in effect through Federal Acquisition Circular 2005 -12. This acquisition is set aside for any small business concerns. The applicable NAICS code for this acquisition is 423450 and the small business size standard is 100. The government intends to award a sole source order to Blood Products Specialties, LLC, doing business as Genesis BPS, 65 Commerce Way, Hackensack, NJ, 07601 for the supplies listed below in accordance with FAR subpart 6.302-1. Eisenhower Army Medical Center, Fort Gordon, GA, has a requirement for the following blood bank equipment: CL IN 0001, 15 each Genesis BPS Part Number 425-CM735A, Genesis Blood Collection Mixer. CLIN 0002, 2 each Manuals for Genesis Blood Collection Mixer. CLIN 0003, 20 each Genesis Battery Packs, Genesis BPS Part Number 425BN122C. No Substitutions Authorized. Terms are FOB Destination-Eisenhower Army Medical Center, Building 300, East Hospital Road, Fort Gordon, GA, 30905-5650. Numbered notes 1 and 22 apply. Required delivery date is 30 days after receipt of order. The provisions of FAR 52.212-1, Instruction s to Offerors-Commercial applies to this acquisition without addenda. The provisions of FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, apply to this acquisition with addenda that direct the use of Wide Area Workflow to submit electro nic invoices if available. Offerors are advised to submit a completed printout of FAR clause 52.212-3, unless the offeror has completed online certifications and representations at the https://orca.bpn.gov/ website.
