- The VA Great Lakes Acquisition Center intends to negotiate a sole source contract with American Medical Depot who is the only authorized distributor in the area for Medtronic USA Inc. to provide ENT Surgical Equipment System and related accessories located at the VAMC North Chicago, 3001 Green Bay Road, North Chicago, IL 60064. The Medtronic M4/Visao Sinus Shaver/Otology Drill System is the only system that has full range rotation of 360 degrees and variable blade speeds. Based upon information received from the using service, to the best of their knowledge, the M4/Visao Sinus Shaver/Otology Drill System and related accessories is the only system that will meet the needs of the facility and has full range rotation of 360 degrees and variable blade speeds. This procurement is being conducted in accordance FAR 6.302-1, ?only one responsible source and no other supplies or services will satisfy the agency requirements?. Contract is firm fixed price and the NAICS Code is 334510. This Notice of Intent is not a request for competitive proposals. No solicitations documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service, to the Contracting Officer within (5) calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Officer, VAMC GLAC, 5000 W. National Ave Bldg. 5, Milwaukee WI 53295 or via fax to (414) 902-5440. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with American Medical Depot.
