- ISSUE: 9/13/06 Classification: 65-Medical, Dental, and Veterinary Equipment and Supplies DUE: 6/21/06 Subject: 65-Radioactive Brachytherapy Seeds DESC: The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Proposal (RFP) in accordance with FAR 12.203 to furnish radiopactive brachytherapy I-125 seeds for delivery from October 2006 through September 2007 with four (4) option years. Items include approximately 4650 seeds loaded in needles either linked or unlinked and 150 calibrated seeds for the Department of Veterans Affairs (VA) Medical Center Radiation Safety Service, Veterans Integrated Network (VISN) 21 facility located at 1000 Locust Street, Reno, NV 89502-2597. Requirements include, but are not limited to furnishing and delivery of radioactive brachytherapy I-125 seeds to the Nuclear Medicine/Radiation Safety Office in strict accordance with the terms and conditions of the solicitation/contract. All seeds are to be shipped to the VASNHCS, Reno in DOT regulated containers/packaging. Initial contract period is October 1, 2006, or date of award if subsequent thereto, through September 30, 2007 with four one-year options through June 30, 2011. Contract award is subject to availability of funds. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract Terms and Conditions to Implement Statutes or Executive Orders - Commercial Items. HARD COPY SOLICITATIONS WILL NOT BE FURNISHED. However, a signed and dated hard copy of Standard Form 1449 must be provided with proposal (via First-Class mail). No telephone or electronic (e-mail) requests for solicitation copies will be accepted. No web-based or faxed proposals will be accepted. All amendments will be posted on the web site. Offerors are advised that they are responsible for checking the web site regularly to obtain amendments.
