***AMENDMENT 01*** THIS PROCUREMENT IS AMENDED TO BE A TOTAL SMALL BUSINESS SET ASIDE. THE CLOSING DATE IS ALSO EXTENDED TO 15 SEP 2006 AT 10:00 AM LOCAL TIME. ALL QUOTES MUST BE IN NLT 10:00 AM TO BE CONSIDERED. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is F2CFMD6207A100 and is issued as a Request for Quotation (RFQ) for Heat software, support and maintenance. The performance period for training and implementation will be coordinated by the end-users. Delivery location, performance and FOB point will be at 25 East Street, D-1, Hickam AFB, HI 96853. Acceptance will be performed by the Government. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-02, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 200502323 and Air Force Acquisition Circular (AFAC) 2005-0209. For informational purposes the North American Industry Classification System code is 511210 Software Publishers. This acquisition will be awarded as a firm-fixed price contract that represents the best value to the government and best shipping time. Contract will consist of twelve (12) line items. 0001 HEAT Software, 35 Each; 0002 Software Maintenance Agreement (SMA)35 Each; 0003 Heat Self service,1 Each; 0004 Heat Self service SMA, 1 Each; 0005 HPK Techs,5 Each; 0006 HPK Techs SMA,5 Each; 0007 HPK HSS (up to 1,000 employees,)1 Each; 0008 HPK HSS SMA (up to 1,000 employees,)1Each; 0009 iHEAT, 45 Each; 0010 IHEAT SMA, 45 Each; 0011 Implementation and Training, 15 Days; 0012 Travel and Expenses, 15 Days This requirement is for the item described above or comparable. Offerors responding to this requirement, must include a specification data sheet on proposed item(s) offered. Offeror may propose other option, and ensure the provision FAR 52.212-3, Offeror Representation and Certifications-Commercial Items (Jan 2005) (this must be completed on http://orca.bpn.gov), DFARS 252.212-7000, Offeror Representation and Certifications-Commercial Items (Nov 1995), and DFARS 252.225-7000, Buy American Act-Balance Payment Program Certificate (Apr 2003) with the quotation. The contractor must be registered in the Central Contractor Registration (CCR) database IAW DFAR 252.204-7004 Required Central Contractor Registration (Nov 2003).
