***AMMENDMENT TO SOLICITATION A SITE VISIT HAS BEEN SCHEDULED FOR 10:00 AM CDT, 19SEP06. CONTRACTORS INTERESTED IN ATTENDING THE SITE VISIT WILL NEED TO REGISTER WITH ONE OF THE POINTS OF CONTACT LISTED IN THIS SOLICITATION BEFORE 04:00 PM, 18SEP06. THE MEETING LOCATION FOR THIS SITE VISIT WILL BE ALTUS AFB SOUTH GATE (LOCATED ON HWY 62). AMMENDMENT TO SOLICITATION*** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F1B3R26247A100 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12 4 Aug 06. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 562991, with annual gross revenue not to exceed $6.5 million. Statement of Work: 1. GENERAL: The contractor will clean, treat and properly dispose of all oil, grease, sand and sludge in the grease traps and oil/water separators that are on Altus Air Force Base, OK. Collection stations are located adjacent to facilities 16, 39, 106, 185, 267, 301, 307, 317, 321, and 349. Contractor will provide all plant, supervision, materials, equipment, tools, vehicles, labor and incidentals to complete cleaning services to the satisfaction of the Contracting Officer. 2. Performance Hours: Clean all listed traps, pits and tanks on a quarterly basis during normal duty hours, between 7:30 AM and 4:30 PM, Monday through Friday. 3. Coordination of Work: Coordinate all cleaning operations with the Quality Assurance Evaluator (QAE) and the Contracting Officer (CO). Submit written work schedules showing areas to be worked, dates of work, and estimated time for completion of each cleaning service to the CO for approval and coordination fifteen (15) days in advance of each quarter?s work to be accomplished. Also, notify the QAE at (580) 481-7755 two (2) work days prior to arrival on the installation. Verification of field measurements and removal quantities is the responsibility of the contractor. 4. Personnel and Safety: Provide only qualified and knowledgeable personnel to perform the tasks required to properly empty and clean grease traps and pits as specified. 4.1. Personnel will wear protective clothing, to include face shields, as necessary during cleaning operations and handling of waste materials. 4.2. Thoroughly wash trap, pit and tank walls, floors/bottoms and tops with pressurized hot water/steam, soap and detergents. Alternate cleaning methods will require submittal for technical review/acceptance and subsequent approval by the Contracting Officer before they are implemented. 4.3. Maintain all equipment, vehicles and holding tanks used in cleaning operations and for hauling waste products in a clean condition before entering the installation and during all extraction operations. 5. Specific Tasks: Remove approximately 6,575 gallons of oil, grease, sludge, mud/sand and water from traps, pits and tanks at the facilities listed below: Commissary, Bldg. 16 Capacity: 990.65 Gal Officer Lounge, Bldg. #39 Capacity: 198.13 Gal Bowling Alley, Bldg. #106 Capacity: 550.00 Gal Base Operations, Bldg. #185 Capacity: 392.30 Gal Fire Dept., Bldg. #267 Capacity: 602.07 Gal Base Chapel, Bldg. 301 Capacity: 648.38 Gal Club Altus, Bldg. 307 Capacity: 190.20 Gal Solar Inn, Bldg. 317 Capacity: 1,902.04 Gal Base Car Wash, Bldg. 321 Capacity: 300.00 Gal Vehicle Maint Car Wash, Bldg. 349 Capacity: 800.00 Gal 5.1. Thoroughly steam clean all traps and pits, adding back water in the trap as needed. 5.2. Deodorize and disinfect all traps, pits and tanks with a soap or a cleansing solution that is environmentally and approved by the CO. 5.3. Ensure containment tanks or vessels are clearly labeled to identify the contents. 5.4. Inspect influent and effluent piping for obstructions or clogs. Report obstructions, immediately, to the QAE. 5.5. Dispose of all grease, oil, sludge, mud/sand and waste water, off the installation, at a certified disposal site. All disposals will comply with existing local, state, and federal regulations. Coordinate with the Altus AFB Environmental Section on handling and disposal of debris to off base disposal locations at (580) 481-7346 or 481-6198. The contractor is responsible for all permits or fees associated with the use of off-base disposal locations. 5.6. Apparent Serviceable Material: Do not dispose of any material, which has obvious value and appears to have been inadvertently placed adjacent to the collection point, unless such material is designated by the QAE for removal. 6. Environmental Compliance: Contractor shall comply with all federal, state, local, and base environmental laws, regulations and policies. The following paragraphs contain information concerning base environmental policies. All letters referenced below are on file at the base contracting office. 6.1. Hazardous Material: All Hazardous Material (HM) brought onto Altus AFB must be registered with the Hazardous Material Pharmacy (HMP), located at the rear of building 228, (580)481-7728. The HM and it?s quantity shall receive a registration number from the HMP prior to transportation onto Altus AFB. Upon arrival, the registration number shall be applied to all containers. Upon completion of service report the used/expended quantity to the HMP along with the disposition of the container. 6.2. Drum Registration Policy: All drums, regardless of size, to include non-regulated material, spare part drums, trash drums, etc., shall receive a registration number prior to transportation onto Altus AFB. The registration number and contents shall be written on two sides of the drum with a paint marker. Registration numbers will be obtained from the HMP, (580) 481-7728. Compliance with AAFBI 32-7001 is mandatory. After use, report the disposition of drum(s) to the HMP. 6.3. Material Safety Data Sheets (MSDS): Maintain MSDS?s for all chemicals in use during the execution of work and ensure MSDS?s are on file with the HMP. 6.4. Flammable Storage Lockers: A list of flammable lockers, their contents, any addition or deletion of contents, and location shall be provided to the HMP. 6.5. Hazardous Waste: Provide proof of personnel Hazardous Waste training to the 97th CES/CEV and the QAE. All Hazardous Waste generated on Altus AFB shall be reported to 97 CES/CEV, Bldg. 396, (580)481-7605 6.6. Toxic Release Inventory: Comply with the reporting requirements for ?toxic? chemicals under the Emergency Planning and Community Right to Know Act (EPCRA). Report quarterly uses of toxic chemicals that are estimated to exceed the yearly ?minimum threshold quantities? contained in the applicable Code of Federal Regulation required under EPCRA, or 5000 pounds, whichever is less. Provide a written report to the 97th CES/CEV and QAE within five working days after the end of each quarter. 6.7. Hazardous Waste Satellite Point Conex/Locker: Any contractor who will accumulate Hazardous Waste, as defined by the applicable Code of Federal Regulation, must establish a satellite accumulation point. The contractor is responsible for providing the conex/locker and all required labels/signs/markings. The accumulation point shall comply with AAFBI 32-7001. Any contractor who must establish a Hazardous Satellite Point Conex/Locker shall contact the 97th CES/CEV for instructions and coordination. 6.8. Spill Reporting: Spills of hazardous material or regulated material such as oils, antifreeze, grease, latex paint, hydraulic fluid, etc., that contaminates either soil, surface water, ground water, or air, shall be reported to 97 CES/CEV immediately for reporting purposes and to implement/direct proper clean up action. Report spills occurring after hours or on weekends to the base fire department, (580)481-6333, and request them to contact the Environmental standby person. 6.9. Used Oil Filters: The contractor shall be responsible for disposal of spent oil and oil filters in accordance with all federal, state, and local laws and regulations. Contractor will be required to supply QAE with receipts as verification. 6.10. Parts Cleaning: Contractor shall comply with existing standards and use only approved parts cleaning solvents. The solvents should be recycled or disposed of in an acceptable manner by the contractor. 6.11. Recycling: Contractor is encouraged to participate in the Altus AFB recycling program for proper disposal of newspaper, plastics, glass, and tin. 7. GOVERNMENT FURNISHED PROPERTY AND SERVICES: All required Air Force forms, regulations, and directives will be furnished by the Government. Initial supplies of these items will be made available from the QAE. 8. CONTRACTOR FURNISHED ITEMS: 8.1. In addition to items specified in Section 6.0, the contractor shall furnish all equipment, parts, fuels, pumps, labor, cleaning material (liquids and non-liquids). No storage site is available on Altus AFB. 8.2. Vehicles: The Contractor shall provide all vehicles required to fulfill the requirements of this contract. All vehicles shall be operated in accordance with base traffic regulations. Vehicles shall be in operable condition and meet local, state, and federal safety requirements. Vehicles shall be painted to present a neat appearance and have the Contractor?s name affixed on both sides of the vehicle. Vehicle repairs on base shall be limited to those minor in nature. 8.3. All grease collection vehicles shall be specifically designed for the purpose to pump out grease trap pit with all attachments needed to clean and deodorize the grease trap pits. 8.4. Container: Contractor shall provide bulk containers. Container shall either be built in the vehicle or as an attachment. Containers shall have a minimum capacity of five hundred (500) gallons and present a neat appearance with proper caps/seals to prevent spillage while under transportation. 9. SLOW MOVING VEHICLES: All vehicles and towed cleaning equipment along with equipment transported over base roads shall be marked by a red/orange reflective triangle visible both day and night from 500 feet. Any load extending over the side of a vehicle shall be flagged. Any load extending 4 feet from the rear or 3 feet from the front of any vehicle or trailer shall be flagged. 10. LEGAL HOLIDAYS: Contractor shall not work on government holidays. 11. INSPECTION: Clean all grease traps, pits or tanks to the requirements of this contract. The Quality Assurance Evaluator (QAE), or Contract Administrator (CA) will inspect for approval for Altus Air Force Base. 12. SAFETY: Work to be performed under this contract is classified as a fire explosion hazard reduction project. 13. FIRE PREVENTION: Contractor shall accomplish work and provide storage of material to comply with Altus Air Force Base Regulation 92-1. 14. SITE CLEAN UP: Ensure each collection site is cleaned and surrounding areas are dressed up after servicing each trap, pit or tank on Altus Air Force Base. (End SOW).
