- Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitat ion; proposals are being requested and a written solicitation will not be issued. The solicitation number is W67K2S 6152-0001 and is issued as a Request for Quotation (RFQ). Quotations shall be received in the contracting office by 4:30 p.m. (MST), Septe mber 13, 2006. The RFQ shall result in a Firm-Fixed Price Contract. This solicitation is 100% small business set-aside. The North American Industry Classification System (NAICS) Code for this acquisition is 453998. The Small Business Size Standard is $ 6.5 Million. The USPFO for Utah has a requirement to purchase Exercise Equipment for various armories in the State of Utah. Equipment will be delivered to one location unless installation is required for warranties to be in effect. Items listed may have different companys model numbers listed. These numbers are given for quoting Brand Name and Equal To items. CLIN 0001. Four (4) sets -500 lb barbell set with each set containing 2 each 7 chrome bars, rubber encased. Troy Barbell or equal State which mfg and model number you are quoting on; CLIN 0002. One (1) each Elliptical Fitness Crosstrainer, Precor EFX576i or equal; CLIN 0003. Three (3) each 4.0 hp commercial treadmills, adjustable bed level and 20 wide bed, pre-programmed workouts and heart rate indicator. Precor ST4600C Super Tread Classic Club or equal; CLIN 0004. One (1) each Leg press machine. 3-way hip and leg sled with linear bearings. Linear Bearings P118 or equal; CLIN 0005. Two (2) each Heavy duty adjustable flat/incline/decl ine weight lifting bench with rubber feet (non-slip). Promaxima FW68 or equal; CLIN 0006. Three (3) each Upright Stationary Bike, Commercial Cardio Advisor Heart Rate Training System, 9 programs with manual, 20 resistance levels, polar heart rate, sel f leveling pads, oversized water bottle, CD Holders, self generating power supply, ISO 9001/9002, Promaxima CV-53U or equal; CLIN 0007. Six (6) each 20-pair dumbbell racks, Two Tier Dumbbell Rack, Promaxima FW15 or equal; CLIN 0008. Four (4) each 10-pa ir dumbbell racks, similar in quality as CLIN 0007, but to hold 10 pair instead of 20; CLIN 0009. One (1) each Sit-up bench. State which mfg and model number you are quoting on; CLIN 0010. One (1) each Pull-up bar, wall mounting. State which mfg and model number you are quoting on; CLIN 0011. One (1) each Curl Bar, 52 Hard chrome Olympic. State which mfg and model number you are quoting on; CLIN 0012. One (1) each Multi-Station weight training system, 4-station multi-stack weight training machin e. To include 18 cable with nylon sheath. Must be compact and include military press, upper body curls, leg press, lat bark ankle strap, leg curls, crunches and heavy weight stack (258 lbs). Precor S3.45 or equal; CLIN 0013. Two (2) pair 15-lb dumbb ells, Chrome; CLIN 0014. Five (5) pair 10-lb dumbbells, Chrome; CLIN 0015. Four (4) each protective floor mats, 24 x 24 inter-locking rubber tile; CLIN 0016. One (1) each 10 x 10 wrestling mat. State which mfg and model number you are quoting on . Items will be delivered to the USPFO for Utah warehouse, 12953 South Minuteman Drive, Draper, UT 84020. The Offerors shall be registered in the DOD Central Contractor Registration (CCR). Information concerning CCR requirements may be viewed via the Internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-888-227-2423. Only contractors registered in the Central Contractor Registration (CCR) can be awarded the contract. Offerors shall have electronic funds transfer (EFT) capabil ity. Offerors proposals shall be valid for a minimum of 30 days to be acknowledged in the offerors proposal. In accordance with FAR52.212-2 Evaluation of Co mmercial Items (Jan 1999) (a), The Government intends to make an award to the Contractor who submits the lowest, technically responsive proposal, and is deemed acceptable and responsible by the Contracting Officer.
