- The Southern AZ VA Health Care System, Surgical Services, has a requirement to enter into a loaner agreement for three (3) Legend Platinum Systems Kit containing the items below. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will NOT be issued. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. Supporting evidence must be furnished in sufficient detail to demonstrate their ability to comply with the below requirements. This notice of intent is not a request for competitive proposals. However, the Government will consider all proposals received within 14 days after date of public action of the synopsis. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular August 2006. The associated NAICS code 339112. The Government contemplates delivery October 1, 2006 FOB origin. The provision at 52.212-1 applies to this acquisition. There are no evaluation criteria as this procurement is being conducted as a sole source, only one source is able to meet the requirement. Item 1 -Each system consists of one (1) each of the following: PM200 Legend Platinum Motor, PC100 Legend Pneumatic Control Unit, PA100-A Legend Lubricant/Diffuser Pack, PA115 Legend Tool Storage Rack, PA400 Legend Inst kit ? din, PA300 Legend 1.2mm Cleaning Brush, PA310 Legend 2.4mm Cleaning Brush, PA320 Legend 3.2mm Cleaning Brush, PA120 Legend ATTACH Cleaning Nozzle, PC110 Legend FN2-DISS to M-SCHR, PC120 Legend FN2-DISS to MA-DISS, PC130 Legend WF4 Connector, 175004EN Legend PM/Service Manual-Eng, 175002EN Legend Service Manual-Eng, PA130 Legend Motor Wrench, 175021ENSP Legend Cleaning Poster: EN/Sp, LIT250006EN Legend Wall Chart. Medtronic Power Surgical Solutions is believed to be the only provider of this type of surgical system with these specifications and capabilities. Medtronic Power Surgical Solutions will provide the above system at no charge with the understanding all Midas Rex Legend Tools and blades are purchased with a 20% discount off the equity price list from Medtronic Power Surgical Solutions. Upon warranty expiration SAVAHCS will receive a 100% discount from the list price for Legend pneumatic motor repair charges. SAVAHCS will not be charge motor repair. Replacement motors will be provided by Midas Rex, FOB origin, in the event of existing equipment failure. Ownership of system will remain with Medtronic Powered Surgical Solutions. This agreement will be in effect for three year period with annual reviews. Either party may terminate agreement at any time with 60 days written notice.
