Description
The Agency for Substance Abuse and Mental Health Services Administration (SAMHSA), Center for Substance Abuse Treatment (CSAT), through the Division of Acquisition Management (DAM), Program Support Center (PSC), is seeking sources for a 9 month contract, using simplified acquisition procedures. The purpose of this contract is for SAMHSA to procure contractual services with organizations having specific expertise in providing technical assistance and training to grassroots faith and community organizations including ATR treatment and recovery provider organizations. Geographical designations are specified. For this particular requisition, the contractor must agree to provide technical assistance and training to faith and community organization including ATR treatment and recovery providers in the states of Tennessee, Connecticut and Florida. The Contractor shall be responsible for but not limited to the following tasks: Task 1 - Meeting with Government Project Officer, Task 1:1 - The contractor shall within five days of the date of the award, meet with the GPO to refine tasks and ensure understanding of all activities. At this meeting or before, the contractor shall provide two copies of a draft work plan and (TA) evaluation plan. Task 1:2 - The contractor shall, within 10 working days following the initial meeting, provide copies of a final work plan, final time lines and schedules, budget proposal and materials/ manuals to be provided. In addition to the first meeting the contractor shall have weekly or bi-weekly to discuss progress to date, issues requiring clarification, problems and next meetings. Task 1:3 - The contractor shall convene three capacity building meetings in each designated state (3 events per state) focusing on infrastructure development, sustainability, entrepreneurship, data reporting (including web based systems) fiscal accounting, marketing, volunteerism and grantmanship. Task 2 - The contractor shall provide a report of each meeting or event. The report will include purpose, attendees and their affiliation, contact information and a follow-up action plan. Reports will be submitted to both the GPO and ATR Lead. Task 2:1 - The contractor shall prepare and submit the GPO weekly status reports, reports of contacts and indicators of successful outcomes. Task 3 - The contractor shall secure acceptable meeting sites and hotel accommodations, provide pre-meeting logistical services, develop and obtain meeting materials, such as registration forms, brochures, invitation letters, agenda, participant lists. GPO shall approve logistical plans, brochures, invitation letter, agenda, participant list, meeting facilitation guidance materials and thank you letters. The contractor shall develop any material not already in existence and provide them to the ATR Lead. Task 4- The contractor shall administer the SAMHSA/GPPRA participant satisfaction and follow-up surveys to evaluate technical assistance activity, input survey data into SAMHSA?s data base and provide a written analysis of survey results (if TA is provided). Task 5 - The contractor shall submit a monthly progress report by the tenth (10th) of the following month. The report shall include meeting survey summaries; summary of action steps taken to execute meetings including selection of presenters, problems encountered and resolutions, description of partnerships and discussion of outcomes; achievements of quantitative goals and objectives; etc.. Task 6 - The contractor shall submit a final report written 30 days after the final series of training activities. The report shall include pre and post customer satisfaction survey summaries; summary of action steps taken to execute meetings, including selection of presenters, problems encountered and their resolution, description of partnerships with other agencies, state government, demographics of participants, discussions of outcomes including descriptive chart representations, participants list and recommendations for future meetings.
