- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is set-aside for small business only. The NAICS Code for this synopsis/solicitation is 334611, Size Standard 500. Solicitation/Purchase Request number F2BTAQ6153A100 is issued as a Request for Quote (RFQ). This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-11 effective 05 July 2006. The Government intends to award the following contract line items: CLIN 0001: SMALL GROUP RECORDING - 2500 Replicated compact disc with 5-color offset label, 2500 Digipak, 4-panel glued into place, C1S stock, 1 disc tray, 2500 assembly with shrink wrap disc label film for 4-color disc Digital hard-copy proof for Digipak, booklet & disc label, the panel folder print is 4/0, the disk art will be 4-color (CMYK). Graphics preflight of supplied artwork files, Graphic files and master CD will be shipped or hand delivered to contractor. Project must be completed by 31 September 2006. FOB: Destination. Inspection and Acceptance: Destination. CLIN 0002: CONCERT BAND RECORDING - - 5000 replicated compact disc with 5 color offset label 5000 CD Digipak, 4-panel glued into place, C1S paper stock, 1 disc tray (clear) with 8-page booklet, 4/4, glued into digipak 5000 assembly with shrink wrap Disc label film for 4-color disc Digital hard-copy proof for Digipak, booklet and disc label, the panel folder print is 4/0, the disk art will be 4-color (CMYK). Graphics preflight of supplied artwork files, graphic files and master CD will be shipped or hand delivered to the contractor. Project must be completed by 31 September 2006. FOB: Destination. The following provisions and clauses apply to this procurement: FAR 52.212-1, Instructions to Offerors-Commercial Items; In accordance with FAR 13.106-1(a)(2) offerors are notified that award will be made on the basis of past performance, delivery, and price. Please attach applicable documentation regarding past performance. Offerors will be evaluated in accordance with FAR 13.106-2. FAR 52.212-3 Offeror Representations and Certification-Commercial Items (all offerors shall include a completed copy of this provision with their proposal); FAR 52.212-4 Contract Terms and Conditions-Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items; paragraphs (b) and (c) the following clauses apply 52.219-6; 52.222-3, 52.225-13, 52.232-33.
