- Description
- The Department of Veterans Affairs, Veterans Integrated Service Network (VISN) 21 is soliciting proposals to provide Dental Laboratory Services for the Livermore Division. The contractor will fabricate complete dentures and removable partial dentures to including picking up of mold from the Dental Clinic at 4951 Arroyo Road, Livermore, CA 94550. The services shall be performed Monday through Friday, 8:00am through 3:30pm, excluding holidays. The anticipated period of performance is from the 10/01/06 through 9/30/07 Mail proposals to Lupe Arroyo, Contracting Officer, VA Palo Alto Health Care System, 3801 Miranda Avenue 90/CCA, Bldg. 6, Room C-136, Palo Alto, CA 94304. All proposals must be received in the issuing office no later than September 5, 2006. No telephonic request will be accepted. Questions regarding this solicitation shall be submitted in writing to lupe.arroyo@va.gov, no later than August 24, 2006 with NO EXCEPTIONS.
