Description
MEDICAL EVACUATION CONTRACT FOR VHA FACILITIES This RFQ VA-101-06-RQ-0132 is intended to procure medical evacuation in matters of National Emergency. This is a combined synopsis/solicitation prepared in accordance with the format in Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document incorporates provisions and clauses in effect through FAC 05-09. The associated NAICS Code work shall be performed under is 481211. This acquisition is unrestricted. The provisions at 52.212-1 Instructions to Offerors-Commercial Items, and 52.212-3 Offeror Representations and Certifications-Commercial Items apply to this solicitation. Offerors are responsible for returning a copy of the Representation and Certifications at FAR 52.212-3 with their proposal. A copy of the provision can be obtained at www.arnet.gov . Commercial items FAR clauses 52.212-4 Contract Terms and Conditions-Commercial Items, and 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items, apply to this acquisition. All Contractors are required to register in the Central Contractor Registration (CCR) database prior to being awarded a contract. Contractors are responsible for the accuracy of information added to CCR. To register go to http://www.ccr.gov/. Statement of Work Background: The intent of this BPA is to secure prompt emergency evacuation of Veteran Affairs patients during emergencies that adversely affect the operations of VA Medical Centers. The infrastructure at Medical Centers may be inoperable due to the emergency and require the timely transport of patients to other Veterans Affairs Medical Centers. Additionally the Department of Veterans Affairs may require the vendor to support the return of patients to the originating Veterans Affairs Medical Center once conditions permit. The services provided will enable United Stated Department of Veterans Affairs to ensure a continuum of medical care to eligible beneficiaries. Requirements: Vendor shall provide or arrange for all necessary transportation and en route medical care to evacuate patients from a Department of Veterans Affairs Medical Center, including necessary medical attendants, equipment, oxygen and supplies. The vendor shall provide services to any United Stated Department of Veterans Affairs Medical Center located throughout the United States where facility operations have been adversely affected by an emergency such as a natural disaster. This includes U.S. Department of Veterans Affairs Medical Centers located in hurricane zones along the U.S. Gulf and East Coasts as well as the Commonwealth of Puerto Rico. Patients shall be transported to no more than three other Veterans Affairs Medical Centers located up to 500 miles distance. The first of these patients shall be transported within six hours of initial notification to the vender and the last of these patients shall be transported within 24 hours of notification. Quantities & Pricing: Pricing shall be provided in units as follows: Transportation and en route care for each critically injured or ill litter borne patient Transportation and en route care for each non-critical litter borne patient Transportation and en route care for each non-critical ambulatory patient Quantities shall be estimated not to exceed the following limits per tasking: 1. 30 critically injured or ill litter borne patients 2. 150 non-critical litter borne patients 3. 100 non-critical ambulatory patients Attachment: Location of Veteran Affairs Medical Centers (Due to size constraints the attachment will be provided as amendment to this solicitation.)
