- Under the authority of Public Law 104-262 and 38 U.S.C. 8153, the VA Medical Center located at 1201 N. W. 16th Street, Miami, Florida, 33125 has a requirement to award a single or multiple award contract for the delivery of Primary Care services under the Advance Clinic Access (ACA) model to veterans of the Department of Veterans Affairs Medical Center, in Homestead, Coral Springs, and Deerfield Beach, Florida, for the period of one calendar (1)year. This contract includes, a two-year renewal option, which may be exercised at the option of the Government subject to the availability of funds. The total duration of the contract will not exceed three (3) years and six months. The Government intends to award indefinite delivery, indefinite quantity, firm fixed price, contract for Primary Care Services. Primary Medical Care Services shall be provided at the Contractor facilities to approximately 3000-6000 eligible veterans per year per location. Contractor(s) shall provide health care providers, medical facilities, medical equipment and supplies, pharmaceutical prescriptions and all administrative functions to fulfill the level of care and range of services as may be required by the VA Medical Center and in accordance with Joint Commission on Accreditation of Healthcare Organizations (JCAHO) or other regulatory agencies. The primary care medical services must be provided in a clinical environment, to veterans in the geographic location(s) covered by this contract. Primary care includes medical evaluation, diagnosis and medically necessary treatment of physiological NAICS code 621498 applies with a size standard of $7.5 million.
