- The 30th Contracting Squadron, Vandenberg AFB, CA intends to award a sole source purchase order with JP Blanchard MD, Inc., 1933 Cliff Dr., Suite 26, Santa Barbara, CA 93109. This acquisition has two CLINs to provide the following: CLIN 0001: The contractor shall provide Histopathology services for the 30th MDG, to include: diagnosis on tissue specimens, phone consultation, detailed written reports, representative slides for quality control, and due to the immediate examination requirements the laboratory must be located within a 1-hour driving radius of the 30th Medical Group. CLIN 0002: The contractor will provide Pathology services for the 30th MDG, to include: a Pathologist with current American Board of Pathology certification, reports to the MTF staff member all information necessary to assure records are maintained correctly IAW JCAHO, OSHA, CAP, CDC, AABB, HSI, and FDA record requirements, and provide data to support both the Performance Improvement Program and the Risk Management process. The applicable NAICS for this acquisition is 621511. The business size is $12.5. The period of performance is from 1 Oct 2006 to 30 Sep 2007. The proposed contract is for items the Government intends to solicit and negotiate with only one source under the Authority of FAR 13.106(b). This notice of intent is not a request for competitive proposals. However, the Government will consider all proposals received within 7 days after date of publication of this synopsis. All questions must be submitted in writing no later than 7 Aug 2006. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
