Description
VA-261-06-RQ-0087 Due: Aug. 18, 2006 Request for Quote (RFQ) - Single/Double Audiometric Exam Suite. The Sierra Pacific Network Consolidated Contracting Activity is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to purchase this equipment for the Sacramento Medical Facility, a division of the Northern California Health Care System. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. The NAICS code is 423450, Employee size standard is 100 employees. Product Service code of 6515 applies. VISN 21 CCA intends to solicit quotations and purchase the following: 1) One (1) RS-252 single/double audiometric exam suite; 2) Cut booth to fit into elevator; 3) Fire panels and appearance package; 4) Shipping and 5) Installation. Offeror shall include warranty information as appropriate with offer. All responses shall be for the following specified manufacture/brand name or equal: Acousti-Medical Instruments RS-252. Offerors should include: 1) Descriptive literature and product number for item offered; 2) Separate unit cost of each item as described above; 3) Evidence of past performance of contracts of similar type, scope, size and complexity that are ongoing or completed with the past three years, references with points of contacts must be identified; 4) Delivery time. The actual RFQ should be posted on or about August 4, 2006. Telephone requests will NOT be accepted. All questions must be submitted in writing, questions may be faxed to W. This is a one time procurement, delivery is requested as soon as possible. Electronic solicitation may be accessed at no cost at www.fedbizopps.gov . Any amendment issued against this solicitation will also be available from the same URL addresses. Co ntractor is responsible for submitting all documents required in accordance with the solicitation, including any amendments which will be posted on this website. Solicitation is also subject to availability of funding. Price includes all parts and labor to repair any failure due to a manufacturer or installation defect; and one site visit during the warranty period for service or repair. Quotations shall be submitted on an all or nothing basis. Partial quotations will be rejected. Quotations shall be valid for 60 days from offer due date.
