- ISSUE: 8/02/06 Classification: J-Maintenance, Repair, and Rebuilding of Equipment. DUE: 8/18/06 Subject: J-Preventive Maintenance and Repair Services for Sterilizers. DESC: The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Quotes (RFQ) in accordance with FAR 12.203 to furnish Preventive Maintenance (PM) and emergency repair service for sterilizer equipment located at the VACNHCS, 1000 Locust Street, Reno, NV 89520-2597 and VACCHCS, 2615 E. Clinton Avenue, Fresno CA 93793. The Contractor shall provide all necessary labor, materials, parts, equipment and travel costs necessary to perform quarterly preventive maintenance (in Nov., Feb., May, and Aug.) and repair service calls for ETO and steam sterilizers and washer equipment listed in Section B, Part I of the solicitation. Contract period is October 1, 2006, or date of award if subsequent thereto, through September 30, 2007 with four one-year options through June 30, 2011. Contract award is subject to availability of funds. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract Terms and Conditions to Implement Statutes or Executive Orders - Commercial Items.
