- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes/offerors are being requested and a written solicitation will not be issued. The USDA Forest Service, Manti-La Sal National Forest, is soliciting quotes/offers for the following requirement: Approximately 2,360 tons of aggregate surfacing that meets Forest Service specifications, grading F, to the Mill Creek Road #50076. The segments receiving aggregate will be staked by the Forest Service and spread lengths will be designated on the ground. Forest Service crews will shape the aggregate. The project can be reached by heading south out of Moab, UT on Highway 191. Proceed approximately 7 miles on Highway 191 to the LaSal Loop Road junction. Take the LaSal Loop Road (paved) for approximately 12.5 miles in a north westerly direction (windy and steep) to the junction of the project road, Mill Creek ? NFSR 50076. The project road is approximately 3.1 miles in length. Offers are due no later than 4:30 P.M. local time, Thursday August 15, 2006 and shall be submitted to: USDA/Forest Service, Ashley National Forest, 355 North Vernal Avenue, Vernal Ut, 84078. The Synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22 and 2001-23. The following FAR provisions apply to this acquisition: 52.212-1, Instruction to Offerors Commercial items, and 52.212-2, Evaluation of Commercial Items, applies to the acquisition. Offerors shall include a completed copy of FAR provision 52.212-3, Offeror Representations and Certifications Commercial Items, and furnish Offeror's Federal Tax Identification Number, Central Contractor Registration Number, DUNS Number. FAR Clauses 52.212-4, Contract Terms and Conditions Commercial Items and 52.212-5, Contract Terms and Conditions Required Implementing Statutes or Executive Orders Commercial Items apply to this acquisition.
