Description
The Agency of Substance Abuse and Mental Health Administration (SAMHSA), Center for Mental Health Services (CMHS), through the General Acquisitions Branch (GAB), Program Support Center (PSC), intends to award a 12 month sole source contract using simplified acquisition procedures to University of Texas Health Science Center at San Antonio, Department of Psychiatry, 7703 Floyd Curl Drive, San Antonio, TX 78229. The anticipated date of award is August 21, 2006. The purpose of this contract is to revise the Medication Management toolkit to be consistent with the lessons learned that were described at CMHS? October 2005 Medication Management meeting, hosted by the NASMHPD Research Institute. The revised version of the fidelity instrument, the revised format used for the first five toolkits in the EBP series, and SAMHSA guidance. The contractor shall complete the following tasks: Provide the GPO with a 5- to 20-page written summary of the lessons learned about implementation of medication management (MM) and use of the MM toolkit to date. Provide the GPO with a work plan for revising the toolkit. Submit the draft revised version of the toolkit to the GPO. Submit a Final Revision Plan to the GPO. The Final Revision Plan shall include (1) a description of feedback on the draft revisions provided by relevant stakeholders, (2) a description of how, when, and by whom feedback was provided, and (3) a detailed description of what additional changes will be made to the revised toolkit based on GPO and other stakeholder feedback. Following approval of the Final Revision Plan by the GPO, submit the final revised version of the toolkit to the GPO. The final revised toolkit must be consistent with the GPO-approved Final Revision Plan. All written portions of the toolkit shall be submitted in MS Word format without graphics or formatting, according to CMHS guidance. The Contractor shall submit the complete revised toolkit in the following formats: -3 clear and complete printed out hard copies, -2 read-only CDs, clearly labeled, -2 editable CDs, clearly labeled, -an outline of each toolkit section that includes all levels of heads. 3 copies of revised videos shall be submitted in read-only DVD format, clearly labeled; an editable version of all revised videos shall also be submitted. Submit to the GPO the following supporting documents: copyright permissions for all previously published or copyrighted material included in the toolkit, including (but not limited to) assessment instruments, forms, figures, graphs, and journal articles consent forms from individuals appearing in video portions of the toolkit, particularly consumers and family members, written acknowledgements of all people involved in producing, reviewing, and revising the MM toolkit, including names, titles, organizational affiliations, and cities and States of residence; names may be listed alphabetically within categories (e.g., development team, consensus panel, independent reviewers, etc.) Schedule of Deliverables: A. Submission of acceptable summary of lessons learned and GPO approval of work plan (2 months after start of contract) B. Submission of acceptable draft revised toolkit (8 month after start of contract) C. Submission of final toolkit, revised in accordance with GPO-approved Final Revision Plan, and all supporting documents (12 months after start of contract). The proposed simplified acquisition is for services for which the Government intends to solicit and negotiate with only one source Under authority of far 6.302. Interested persons may identify their interest by responding to the requirement within 10 days after the date of publication of this notice. A determination by the government not to compete this proposed simplified acquisition based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. No solicitation is available.
